# Linagliptin
## Overview
Linagliptin is a dipeptidyl peptidase-4 (DPP-4) inhibitor that increases incretin hormone concentrations, stimulating glucose-dependent insulin release and decreasing glucagon levels. It is unique among DPP-4 inhibitors as its primary route of elimination is biliary/fecal, requiring no dose adjustments for renal or hepatic impairment.
## Primary Indications
Type 2 diabetes mellitus (as an adjunct to diet and exercise to improve glycemic control).
## Adult Dosing
* **Standard Dose:** 5 mg orally once daily.
* **Maximum Dose:** 5 mg/day.
## Pediatric Dosing
* **Safety and Efficacy:** Not established for patients under 18 years of age.
* **Clinical Note:** DPP-4 inhibitors are generally not first-line therapy for pediatric type 2 diabetes.
## Dose Adjustments
* **Renal Impairment:** No dose adjustment required regardless of CrCl.
* **Hepatic Impairment:** No dose adjustment required.
## Contraindications
* Hypersensitivity to linagliptin (e.g., anaphylaxis, angioedema, exfoliative skin conditions).
## Adverse Effects
* **Common:** Nasopharyngitis, hypoglycemia (when used with insulin or secretagogues, e.g., sulfonylureas).
* **Serious:** Acute pancreatitis (discontinue if suspected), severe and disabling arthralgia, bullous pemphigoid, hypersensitivity reactions.
## Key Drug Interactions
* **Strong P-gp/CYP3A4 Inducers (e.g., Rifampin):** May decrease linagliptin efficacy. Avoid concurrent use if possible; increase monitoring if combined.
* **Insulin/Sulfonylureas:** Increased risk of hypoglycemia; consider dose reduction of the insulin secretagogue.
## Monitoring
* **HbA1c:** Every 3–6 months.
* **Glucose:** Self-monitoring of blood glucose (SMBG).
* **Clinical:** Monitor for signs/symptoms of pancreatitis (persistent, severe abdominal pain) and bullous pemphigoid (blistering or erosions).
## Clinical Pearls
* **PK Profile:** Linagliptin exhibits non-linear protein binding. Its pharmacokinetic profile is minimally affected by renal function, making it a preferred agent for patients with advanced chronic kidney disease or dialysis.
* **Administration:** May be taken with or without food.
* **Weight/BP:** Weight-neutral and generally does not affect blood pressure.
* **Combination:** Often available as a fixed-dose combination with metformin (Jentadueto) or empagliflozin (Glyxambi).
