# Linagliptin
## Overview
Linagliptin is a dipeptidyl peptidase-4 (DPP-4) inhibitor that increases incretin hormone concentrations, stimulating glucose-dependent insulin release and decreasing glucagon levels. It is unique among DPP-4 inhibitors for its primary biliary excretion rather than renal.
## Primary Indications
Adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus.
## Adult Dosing
* **Standard Dose:** 5 mg orally once daily.
* **Maximum Dose:** 5 mg daily.
## Pediatric Dosing
* **Safety/Efficacy:** Not established for use in pediatric patients under 18 years of age.
## Dose Adjustments
* **Renal Impairment:** No dose adjustment required regardless of renal function (eGFR).
* **Hepatic Impairment:** No dose adjustment required.
## Contraindications
* History of hypersensitivity reaction to linagliptin (e.g., anaphylaxis, angioedema, exfoliative skin conditions).
* Type 1 diabetes or diabetic ketoacidosis.
## Adverse Effects
* **Common:** Nasopharyngitis, increased uric acid, hyperlipidemia.
* **Serious:** Acute pancreatitis (discontinue if suspected), bullous pemphigoid, severe arthralgia, hypersensitivity reactions.
## Key Drug Interactions
* **Strong CYP3A4/P-gp Inducers:** (e.g., rifampin). Co-administration may reduce linagliptin efficacy; consider alternative therapy if glycaemic control is inadequate.
* **Insulin/Sulfonylureas:** Risk of hypoglycemia increases; consider dose reduction of the insulin secretagogue.
## Monitoring
* **HbA1c:** Every 3 to 6 months.
* **Pancreatitis:** Monitor for persistent, severe abdominal pain.
* **Skin:** Monitor for blistering or erosions if bullous pemphigoid is suspected.
## Clinical Pearls
* Linagliptin is "renally safe" because it is excreted primarily through bile and the gastrointestinal tract, making it a preferred choice in patients with chronic kidney disease (CKD) or ESRD.
* Can be taken with or without food.
* Does not typically cause weight gain.
* If a dose is missed, take as soon as remembered, but do not double the dose.
