# Linagliptin
## Overview
Linagliptin is a dipeptidyl peptidase-4 (DPP-4) inhibitor that increases incretin hormone concentrations, lowering fasting and postprandial glucose levels in a glucose-dependent manner. Unlike other DPP-4 inhibitors, it is primarily excreted via the hepatobiliary system.
## Primary Indications
Type 2 diabetes mellitus as an adjunct to diet and exercise to improve glycemic control.
## Adult Dosing
5 mg orally once daily. May be taken with or without food.
## Pediatric Dosing
Safety and effectiveness in pediatric patients (under 18 years) have not been established.
## Dose Adjustments
* **Renal Impairment:** No dosage adjustment indicated; safe for use in patients with severe renal impairment or end-stage renal disease.
* **Hepatic Impairment:** No dosage adjustment required.
## Contraindications
* History of hypersensitivity reaction to linagliptin (e.g., anaphylaxis, angioedema, exfoliative skin conditions).
## Adverse Effects
* **Common:** Nasopharyngitis, hypoglycemia (when used with secretagogues or insulin), hyperuricemia.
* **Serious:** Pancreatitis (post-marketing reports), bullous pemphigoid, severe arthralgia, hypersensitivity reactions.
## Key Drug Interactions
* **Strong P-glycoprotein/CYP3A4 inducers (e.g., rifampin):** May decrease linagliptin efficacy. If combined, monitor glycemic control closely; increasing the dose of linagliptin is not recommended due to fixed dosing.
## Monitoring
* **HbA1c:** Assess response typically every 3–6 months.
* **Pancreatitis:** Monitor for persistent, severe abdominal pain.
* **Bullous Pemphigoid:** Monitor for blistering or erosions.
## Clinical Pearls
* Linagliptin is unique among DPP-4 inhibitors because it does not require renal dose adjustments, making it a preferred choice in patients with chronic kidney disease.
* It does not generally cause weight gain.
* If a dose is missed, take it as soon as remembered, but do not take two doses on the same day.
* As with all incretin mimetics, assess for signs/symptoms of pancreatitis and discontinue if suspected.
