# Linagliptin
## Overview
Linagliptin is a dipeptidyl peptidase-4 (DPP-4) inhibitor that increases incretin hormone concentrations, stimulating glucose-dependent insulin release and decreasing glucagon levels. Unlike most agents in its class, it is primarily excreted via the biliary/gastrointestinal tract.
## Primary Indications
Type 2 diabetes mellitus as an adjunct to diet and exercise to improve glycemic control.
## Adult Dosing
5 mg orally once daily. May be taken with or without food.
## Pediatric Dosing
Safety and effectiveness in pediatric patients (under 18 years) have not been established.
## Dose Adjustments
* **Renal Impairment:** No dose adjustment required.
* **Hepatic Impairment:** No dose adjustment required.
* **Geriatric:** No dose adjustment required.
## Contraindications
* History of hypersensitivity reaction to linagliptin (e.g., anaphylaxis, angioedema, exfoliative skin conditions).
## Adverse Effects
* **Common:** Nasopharyngitis, increased uric acid.
* **Serious:** Acute pancreatitis (discontinue if suspected), severe and disabling arthralgia, bullous pemphigoid, hypersensitivity reactions.
## Key Drug Interactions
* **Strong P-glycoprotein/CYP3A4 Inducers:** (e.g., rifampin) May decrease linagliptin efficacy. If combined, monitor glycemic control; consider an alternative agent if control is inadequate.
* **Insulin/Insulin Secretagogues:** (e.g., sulfonylureas) May increase the risk of hypoglycemia. A lower dose of the secretagogue may be required.
## Monitoring
* **HbA1c:** Typically every 3–6 months.
* **Pancreatitis:** Monitor for unexplained, severe abdominal pain (with or without vomiting).
* **Skin/Joints:** Monitor for new or worsening blistering or severe joint pain.
## Clinical Pearls
* Linagliptin is unique among DPP-4 inhibitors as its primary route of elimination is non-renal, making it a preferred choice for patients with significant renal impairment.
* Does not cause weight gain.
* It is not recommended for use in patients with Type 1 diabetes.
* The clinical relevance of the interaction with rifampin is often mitigated by the drug's wide therapeutic index, but monitoring remains appropriate.
