# Linagliptin
## Overview
Linagliptin is a dipeptidyl peptidase-4 (DPP-4) inhibitor that increases incretin hormone concentrations, leading to glucose-dependent insulin release and reduced glucagon levels. It is unique among DPP-4 inhibitors due to its non-renal primary route of elimination.
## Primary Indications
Adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus.
## Adult Dosing
* **Standard dose:** 5 mg orally once daily.
* **Maximum dose:** 5 mg daily.
## Pediatric Dosing
Safety and effectiveness have not been established in patients younger than 18 years. Use is generally not recommended in the pediatric population.
## Dose Adjustments
* **Renal Impairment:** No dose adjustment required.
* **Hepatic Impairment:** No dose adjustment required.
## Contraindications
* History of hypersensitivity reaction to linagliptin (e.g., anaphylaxis, angioedema, exfoliative skin conditions).
* Type 1 diabetes or treatment of diabetic ketoacidosis.
## Adverse Effects
* **Common:** Nasopharyngitis, hypoglycemia (when used with secretagogues or insulin), hyperuricemia.
* **Serious:** Acute pancreatitis (discontinue if suspected), severe and disabling arthralgia, hypersensitivity reactions, bullous pemphigoid.
## Key Drug Interactions
* **Strong P-gp/CYP3A4 Inducers (e.g., Rifampin):** May decrease linagliptin efficacy. Avoid co-administration if possible.
* **Insulin/Insulin Secretagogues (e.g., Sulfonylureas):** Increased risk of hypoglycemia; a lower dose of the secretagogue or insulin may be required.
## Monitoring
* **Glycemic Control:** HbA1c (typically every 3–6 months), periodic fasting plasma glucose.
* **Pancreatitis:** Monitor for persistent, severe abdominal pain.
* **Skin:** Proactively monitor for blistering/erosions suggestive of bullous pemphigoid.
## Clinical Pearls
* Linagliptin is primarily excreted via the bile and gut; it is the only DPP-4 inhibitor that does not typically require dosage adjustments in patients with renal impairment.
* It can be taken with or without food.
* If a dose is missed, take it as soon as remembered, but do not take two doses on the same day.
* Does not cause weight gain.
