# Linagliptin
## Overview
Linagliptin is a dipeptidyl peptidase-4 (DPP-4) inhibitor that increases incretin hormone concentrations, stimulating glucose-dependent insulin release and decreasing glucagon levels. It is unique among DPP-4 inhibitors for its primary biliary/enteric excretion.
## Primary Indications
Type 2 diabetes mellitus (T2DM) as an adjunct to diet and exercise to improve glycemic control.
## Adult Dosing
5 mg orally once daily. May be taken with or without food.
## Pediatric Dosing
Safety and effectiveness in pediatric patients (under 18 years) have not been established.
## Dose Adjustments
* **Renal Impairment:** No dose adjustment required.
* **Hepatic Impairment:** No dose adjustment required.
* **Geriatric:** No dose adjustment required.
## Contraindications
* History of hypersensitivity to linagliptin.
* Type 1 diabetes or treatment of diabetic ketoacidosis (not effective).
## Adverse Effects
* **Common:** Nasopharyngitis.
* **Serious:** Acute pancreatitis (discontinue if suspected), severe and disabling arthralgia, hypersensitivity reactions (anaphylaxis, angioedema, exfoliative skin conditions), bullous pemphigoid.
## Key Drug Interactions
* **Strong CYP3A4/P-gp Inducers (e.g., rifampin):** May decrease linagliptin efficacy if co-administered long-term. Consider alternative agents if glycemic control is inadequate.
* **Insulin/Secretagogues:** Increased risk of hypoglycemia when used in combination; consider dose reduction of the secretagogue or insulin.
## Monitoring
* HbA1c (typically every 3–6 months).
* Assess for new or worsening joint pain.
* Monitor for signs/symptoms of pancreatitis (persistent, severe abdominal pain radiating to the back, vomiting).
* Monitor for skin blisters or erosions (bullous pemphigoid).
## Clinical Pearls
* Linagliptin is the only DPP-4 inhibitor that does not require dose adjustment for renal impairment, making it a preferred choice in patients with chronic kidney disease (CKD).
* It does not cause weight gain and has a low risk of hypoglycemia when used as monotherapy.
* The half-life is approximately 100–130 hours due to extensive binding to DPP-4 enzymes, but the 5 mg once-daily dose provides sufficient coverage.
