# Linagliptin
## Overview
Linagliptin is a dipeptidyl peptidase-4 (DPP-4) inhibitor that increases incretin hormone concentrations, stimulating glucose-dependent insulin release and decreasing glucagon levels. It is characterized by high protein binding and predominant excretion via the bile/gastrointestinal tract.
## Primary Indications
Type 2 diabetes mellitus (T2DM) as an adjunct to diet and exercise to improve glycemic control.
## Adult Dosing
Standard dose: 5 mg orally once daily.
Maximum dose: 5 mg/day.
## Pediatric Dosing
Safety and efficacy have not been established in patients younger than 18 years. Use is generally not recommended in the pediatric population.
## Dose Adjustments
* **Renal Impairment:** No dose adjustment required.
* **Hepatic Impairment:** No dose adjustment required.
* **Elderly:** No dose adjustment required based on age.
## Contraindications
* History of serious hypersensitivity reactions (e.g., anaphylaxis, angioedema, exfoliative skin conditions) to linagliptin.
* Type 1 diabetes or treatment of diabetic ketoacidosis.
## Adverse Effects
* **Common:** Nasopharyngitis, hypoglycemia (when used with secretagogues or insulin).
* **Serious:** Acute pancreatitis (post-marketing reports), severe and disabling arthralgia, bullous pemphigoid, and hypersensitivity reactions.
## Key Drug Interactions
* **Strong CYP3A4/P-gp Inducers (e.g., rifampin, carbamazepine, phenytoin):** May decrease linagliptin efficacy. If co-administration is necessary, monitor glycemic control closely.
* **Insulin/Insulin Secretagogues (e.g., sulfonylureas):** Increased risk of hypoglycemia; a lower dose of the secretagogue or insulin may be required.
## Monitoring
* **HbA1c:** Checked at least twice yearly in patients who meet treatment goals and quarterly in those not meeting goals.
* **Renal Function:** While no dose adjustment is needed, baseline and periodic monitoring are standard practice for diabetes management.
* **Clinical:** Monitor for signs of pancreatitis (persistent, severe abdominal pain) or bullous skin lesions.
## Clinical Pearls
* **Excretion:** Unlike other DPP-4 inhibitors (e.g., sitagliptin, saxagliptin), linagliptin does not rely on renal clearance, making it a preferred choice for patients with significant renal impairment.
* **Administration:** May be taken with or without food.
* **Effectiveness:** Linagliptin is considered weight-neutral and has a low intrinsic risk of hypoglycemia when used as monotherapy.
* **Heart Failure:** Use caution in patients with a history of heart failure; some studies with other DPP-4 inhibitors (specifically saxagliptin) suggested an increased risk of hospitalization for heart failure, though this class effect is debated.
