# Linagliptin
## Overview
Linagliptin is a dipeptidyl peptidase-4 (DPP-4) inhibitor that increases incretin hormone concentrations, stimulating glucose-dependent insulin release and decreasing glucagon levels. It is unique among DPP-4 inhibitors for its primarily biliary excretion, allowing use in renal impairment without dose adjustment.
## Primary Indications
Adjunct to diet and exercise to improve glycemic control in patients with type 2 diabetes mellitus.
## Adult Dosing
* **Standard Dose:** 5 mg orally once daily.
* **Maximum Dose:** 5 mg daily.
## Pediatric Dosing
* **Safety/Efficacy:** Not established for pediatric populations (<18 years). Use is not recommended.
## Dose Adjustments
* **Renal Impairment:** No dose adjustment required (regardless of severity, including ESRD).
* **Hepatic Impairment:** No dose adjustment required.
* **Concomitant use with Insulin/Secretagogues:** Consider a lower dose of the insulin secretagogue or insulin to reduce the risk of hypoglycemia.
## Contraindications
* History of serious hypersensitivity reactions (e.g., anaphylaxis, angioedema, exfoliative skin conditions) to linagliptin.
## Adverse Effects
* **Common:** Nasopharyngitis, diarrhea, arthralgia.
* **Serious:** Acute pancreatitis (discontinue if symptoms occur), severe/disabling arthralgia, bullous pemphigoid, and hypersensitivity reactions.
## Key Drug Interactions
* **Strong CYP3A4/P-gp Inducers (e.g., Rifampin):** May decrease linagliptin exposure. Increased monitoring of glycemic control or use of an alternative agent may be necessary.
## Monitoring
* **Glycemic Control:** HbA1c (typically every 3–6 months).
* **Clinical:** Monitor for signs/symptoms of pancreatitis (persistent severe abdominal pain) and bullous pemphigoid (blistering or erosions).
## Clinical Pearls
* Linagliptin is weight-neutral and has a low intrinsic risk of hypoglycemia when used as monotherapy.
* It is not indicated for the treatment of type 1 diabetes or diabetic ketoacidosis.
* Unlike other DPP-4 inhibitors, no renal dosing adjustment is required, making it a preferred choice for patients with significant chronic kidney disease.
