# Linagliptin
## Overview
Linagliptin is a dipeptidyl peptidase-4 (DPP-4) inhibitor that increases incretin hormone concentrations, stimulating glucose-dependent insulin release and decreasing glucagon levels. It is unique among DPP-4 inhibitors as it is primarily excreted via the biliary/gastrointestinal route rather than the kidneys.
## Primary Indications
Type 2 diabetes mellitus (as an adjunct to diet and exercise to improve glycemic control).
## Adult Dosing
* **Standard dose:** 5 mg orally once daily.
* **Maximum dose:** 5 mg daily.
## Pediatric Dosing
Safety and efficacy have not been established in pediatric patients (under 18 years of age).
## Dose Adjustments
* **Renal impairment:** No dose adjustment required.
* **Hepatic impairment:** No dose adjustment required.
* **Geriatric:** No dose adjustment required based on age.
## Contraindications
* History of hypersensitivity reaction to linagliptin (e.g., anaphylaxis, angioedema, exfoliative skin conditions).
* Type 1 diabetes or diabetic ketoacidosis (not effective).
## Adverse Effects
* **Common:** Nasopharyngitis, hypoglycemia (when used with secretagogues or insulin).
* **Serious:** Acute pancreatitis (rare), severe and disabling arthralgia, bullous pemphigoid, hypersensitivity reactions.
## Key Drug Interactions
* **CYP3A4/P-gp Inducers (e.g., Rifampin):** May decrease linagliptin concentrations. If a potent inducer must be used, consider an alternative to linagliptin or monitor glycemic control closely.
* **Insulin/Sulfonylureas:** Increased risk of hypoglycemia; a dose reduction of the insulin or insulin secretagogue may be necessary.
## Monitoring
* **HbA1c:** Checked at least twice yearly in patients meeting goals, or quarterly if not at goal.
* **Pancreatitis:** Monitor for persistent, severe abdominal pain.
* **Skin:** Monitor for blistering or erosions suggestive of bullous pemphigoid.
## Clinical Pearls
* **Unique excretion:** Linagliptin is the only oral DPP-4 inhibitor that does not require renal dose adjustment, making it preferred for patients with chronic kidney disease.
* **Weight neutrality:** Linagliptin is generally weight-neutral.
* **Administration:** May be taken with or without food.
* **Missed dose:** If a dose is missed, take it as soon as remembered, but do not take two doses on the same day.
