# Linagliptin
## Overview
Linagliptin is a dipeptidyl peptidase-4 (DPP-4) inhibitor that increases incretin hormone concentrations, stimulating glucose-dependent insulin release and decreasing glucagon levels. It is unique among DPP-4 inhibitors for being primarily excreted via the enterohepatic system rather than the kidneys.
## Primary Indications
Adjunctive therapy to diet and exercise to improve glycemic control in patients with type 2 diabetes mellitus.
## Adult Dosing
5 mg orally once daily. May be taken with or without food.
## Pediatric Dosing
Safety and efficacy have not been established in pediatric patients (under 18 years). Use is not recommended.
## Dose Adjustments
* **Renal Impairment:** No dose adjustment required.
* **Hepatic Impairment:** No dose adjustment required.
* **Geriatric:** No dose adjustment required.
## Contraindications
* History of hypersensitivity reaction to linagliptin (e.g., angioedema, urticaria, or bronchial hyperreactivity).
* History of pancreatitis.
## Adverse Effects
* **Common:** Nasopharyngitis, diarrhea, arthralgia.
* **Serious:** Acute pancreatitis (discontinue immediately if suspected), bullous pemphigoid (may require topical or systemic immunosuppressive therapy), severe hypersensitivity reactions, potential for severe joint pain.
## Key Drug Interactions
* **Strong CYP3A4/P-gp Inducers (e.g., Rifampin):** May decrease linagliptin efficacy. If combined, monitor glycemic control closely as dose may need adjustment or therapy may require switching to a non-CYP3A4-metabolized agent.
* **Insulin/Secretagogues:** May increase the risk of hypoglycemia; a reduction in the dose of the insulin or secretagogue may be required.
## Monitoring
* **HbA1c:** Typically every 3–6 months.
* **Pancreatitis:** Monitor for persistent, severe abdominal pain.
* **Skin:** Monitor for blistering or erosions suggestive of bullous pemphigoid.
* **Renal Function:** While dose adjustment is not required, monitoring is standard practice in diabetic care.
## Clinical Pearls
* **Renal Sparing:** Linagliptin is the only oral DPP-4 inhibitor that does not require dose adjustment for renal impairment.
* **Weight Neutral:** Linagliptin is weight neutral and has a low risk of hypoglycemia when used as monotherapy.
* **Not for Type 1 DM:** Should not be used for the treatment of type 1 diabetes or diabetic ketoacidosis.
