# Linagliptin
## Overview
Linagliptin is a dipeptidyl peptidase-4 (DPP-4) inhibitor that increases incretin hormone concentrations, stimulating glucose-dependent insulin release and decreasing glucagon levels. It is unique among DPP-4 inhibitors for being primarily excreted via the hepatobiliary system rather than renally.
## Primary Indications
Adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus (T2DM).
## Adult Dosing
Standard dose: 5 mg orally once daily.
Maximum dose: 5 mg daily.
May be taken with or without food.
## Pediatric Dosing
Safety and effectiveness in pediatric patients under 18 years of age have not been established. Use is generally not recommended in this population.
## Dose Adjustments
* **Renal Impairment:** No dose adjustment required.
* **Hepatic Impairment:** No dose adjustment required.
* **Concomitant Use:** If adding a potent P-glycoprotein or CYP3A4 inducer (e.g., rifampin), clinical response should be monitored, as linagliptin efficacy may be reduced.
## Contraindications
Hypersensitivity to linagliptin (e.g., anaphylaxis, angioedema, urticaria, or bronchial hyperreactivity). History of pancreatitis (though clinical trial evidence remains debated, caution is advised).
## Adverse Effects
* **Common:** Nasopharyngitis, hypoglycemia (when used with secretagogues or insulin), and hyperuricemia.
* **Serious:** Acute pancreatitis (post-marketing reports), bullous pemphigoid (may require discontinuation), severe arthralgia, and hypersensitivity reactions.
## Key Drug Interactions
* **Strong CYP3A4/P-gp Inducers (e.g., Rifampin):** May decrease linagliptin exposure, potentially reducing glycemic efficacy.
* **Insulin/Sulfonylureas:** May increase the risk of hypoglycemia; a dose reduction of the insulin or insulin secretagogue may be necessary.
## Monitoring
* **HbA1c:** Baseline, then every 3–6 months.
* **Renal Function:** Generally not required for dosing, but appropriate for general T2DM management.
* **Clinical:** Monitor for signs of pancreatitis (persistent severe abdominal pain) and bullous pemphigoid (blistering or erosions).
## Clinical Pearls
* **Renal Advantage:** Unique among DPP-4 inhibitors as it does not require dose adjustment in any stage of chronic kidney disease or end-stage renal disease.
* **Pancreatitis:** Patients should be advised to discontinue the medication and seek medical attention if they experience persistent, severe abdominal pain.
* **Hypoglycemia:** When used as monotherapy, the risk of hypoglycemia is low; however, the risk significantly increases when combined with insulin or sulfonylureas.
