# Tabgenixim (Cefixime)
## Overview
Tabgenixim is a brand name for cefixime, a third-generation oral cephalosporin antibiotic. It acts by inhibiting bacterial cell wall synthesis. It is characterized by high stability in the presence of beta-lactamases compared to earlier cephalosporins.
## Primary Indications
* Uncomplicated urinary tract infections (UTI).
* Otitis media.
* Pharyngitis and tonsillitis.
* Acute bronchitis and acute exacerbations of chronic bronchitis.
* Uncomplicated gonorrhea (cervical, urethral, and rectal).
## Adult Dosing
* **General infections:** 400 mg once daily or 200 mg every 12 hours.
* **Uncomplicated gonorrhea:** 400 mg as a single oral dose.
* **Maximum:** 400 mg/day for general infections.
## Pediatric Dosing
* **General infections (Children > 6 months):** 8 mg/kg/day administered as a single daily dose or divided into two doses (4 mg/kg every 12 hours).
* **Otitis Media:** 8 mg/kg/day once daily or 4 mg/kg twice daily.
* **Maximum:** Do not exceed the adult maximum of 400 mg/day.
## Dose Adjustments
* **Renal Impairment:**
* Creatinine Clearance (CrCl) 21–60 mL/min: Reduce dose by 25%.
* CrCl < 20 mL/min: Reduce dose by 50%.
* **Hepatic Impairment:** No specific dosage adjustment required.
## Contraindications
* Known hypersensitivity to cefixime or other cephalosporin antibiotics.
* History of severe allergic reactions (anaphylaxis) to penicillins or other beta-lactams.
## Adverse Effects
* **Common:** Diarrhea (often dose-dependent), abdominal pain, nausea, dyspepsia.
* **Serious:** *Clostridioides difficile*-associated diarrhea, hypersensitivity reactions (rash, urticaria, serum sickness), and rare cases of Stevens-Johnson syndrome.
## Key Drug Interactions
* **Anticoagulants:** May increase prothrombin time/INR. Monitor patients on warfarin closely.
* **Probenecid:** May increase serum concentrations of cefixime by inhibiting renal excretion.
* **Carbamazepine:** Potential increases in carbamazepine levels when taken concurrently.
## Monitoring
* Monitor for signs of hypersensitivity or anaphylaxis during the first dose.
* Assess for persistent diarrhea (potential *C. difficile* infection).
* In prolonged therapy, assess renal and hepatic function.
## Clinical Pearls
* Cefixime has poor activity against *Staphylococcus aureus* or *Enterococci*.
* Diarrhea is more common with the suspension formulation than with tablets.
* The medication can be taken with or without food.
