# Linagliptin
## Overview
Linagliptin is a dipeptidyl peptidase-4 (DPP-4) inhibitor that increases incretin hormone activity, stimulating glucose-dependent insulin release and decreasing glucagon levels. It is unique among DPP-4 inhibitors for its primarily biliary excretion, allowing for clinical use without dose adjustments in renal impairment.
## Primary Indications
Adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus.
## Adult Dosing
* **Standard Dose:** 5 mg orally once daily.
* **Maximum Dose:** 5 mg daily.
## Pediatric Dosing
* **Safety and Efficacy:** Not established in pediatric patients (<18 years). Use is not recommended.
## Dose Adjustments
* **Renal Impairment:** No dose adjustment necessary regardless of stage (including ESRD).
* **Hepatic Impairment:** No dose adjustment necessary.
## Contraindications
* History of a serious hypersensitivity reaction to linagliptin (e.g., anaphylaxis, angioedema, exfoliative skin conditions).
## Adverse Effects
* **Common:** Nasopharyngitis, hypoglycemia (when used with insulin or secretagogues).
* **Serious:** Acute pancreatitis (discontinue if suspected), bullous pemphigoid, severe arthralgia, hypersensitivity reactions.
## Key Drug Interactions
* **Strong CYP3A4/P-gp Inducers (e.g., rifampin):** May decrease linagliptin efficacy. If possible, avoid concomitant use.
* **Insulin/Sulfonylureas:** Increased risk of hypoglycemia; consider down-titration of the concomitant insulin or secretagogue when initiating linagliptin.
## Monitoring
* **Glycemic Control:** HbA1c (typically every 3–6 months), fasting plasma glucose.
* **Pancreatitis:** Monitor for signs of severe, persistent abdominal pain.
* **Skin:** Monitor for blistering/erosive lesions suggestive of bullous pemphigoid.
## Clinical Pearls
* **Renal Advantage:** Often preferred in patients with chronic kidney disease because it does not require dosage titration for creatinine clearance.
* **Administration:** Can be taken with or without food.
* **Weight Influence:** Linagliptin is considered weight-neutral.
* **Mechanism:** Does not cause hypoglycemia as monotherapy; hypoglycemia risk arises only with combination therapy involving insulin or sulfonylureas.
