# Linagliptin
## Overview
Linagliptin is a dipeptidyl peptidase-4 (DPP-4) inhibitor that increases incretin hormone concentrations, stimulating glucose-dependent insulin release and decreasing glucagon levels. It is unique among DPP-4 inhibitors for being primarily excreted via the non-renal route (biliary).
## Primary Indications
Type 2 diabetes mellitus as an adjunct to diet and exercise to improve glycemic control.
## Adult Dosing
* **Standard dose:** 5 mg orally once daily.
* **Maximum dose:** 5 mg daily.
## Pediatric Dosing
Safety and effectiveness in pediatric patients (younger than 18 years) have not been established.
## Dose Adjustments
* **Renal impairment:** No dosage adjustment is required regardless of renal function.
* **Hepatic impairment:** No dosage adjustment is required.
* **Geriatric:** No dosage adjustment indicated based on age.
## Contraindications
* History of a serious hypersensitivity reaction to linagliptin (e.g., anaphylaxis, angioedema, exfoliative skin conditions).
## Adverse Effects
* **Common:** Nasopharyngitis, hypoglycemia (when used with insulin or secretagogues).
* **Serious:** Acute pancreatitis, severe/disabling arthralgia, bullous pemphigoid, and hypersensitivity reactions.
## Key Drug Interactions
* **Strong CYP3A4/P-gp inducers (e.g., rifampin, carbamazepine, phenytoin):** May decrease linagliptin efficacy. If combined, monitor glycemic control closely.
* **Insulin/Sulfonylureas:** Increased risk of hypoglycemia; a lower dose of the secretagogue or insulin may be required.
## Monitoring
* **Glycemic control:** HbA1c (typically every 3–6 months), fasting plasma glucose.
* **Signs/Symptoms:** Monitor for symptoms of pancreatitis (persistent severe abdominal pain) and bullous pemphigoid (blistering/erosions).
## Clinical Pearls
* **Renal safety:** Linagliptin is the preferred DPP-4 inhibitor in patients with severe renal impairment or end-stage renal disease because it does not require dose reduction.
* **Administration:** May be taken with or without food.
* **Weight:** Linagliptin is weight-neutral.
* **Heart Failure:** While saxagliptin has a specific warning, the SAVOR-TIMI 53 study did not show an increased risk of heart failure hospitalization with linagliptin (CARMELINA trial).
