# Linagliptin
## Overview
Linagliptin is a dipeptidyl peptidase-4 (DPP-4) inhibitor that increases incretin hormone levels (GLP-1 and GIP), which enhance insulin secretion and decrease glucagon secretion in a glucose-dependent manner. Unlike other DPP-4 inhibitors, it is primarily excreted via the biliary/gastrointestinal route rather than the kidneys.
## Primary Indications
Adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus.
## Adult Dosing
* **Standard Dose:** 5 mg orally once daily.
* **Maximum Dose:** 5 mg daily.
## Pediatric Dosing
* **Safety/Efficacy:** Not established for use in pediatric patients under 18 years of age.
## Dose Adjustments
* **Renal Impairment:** No dosage adjustment necessary for patients with renal impairment (mild, moderate, or severe).
* **Hepatic Impairment:** No dosage adjustment necessary.
## Contraindications
* Known hypersensitivity to linagliptin (e.g., anaphylaxis, angioedema, urticaria).
## Adverse Effects
* **Common:** Nasopharyngitis, hypoglycemia (when used with insulin or sulfonylureas).
* **Serious:** Acute pancreatitis (discontinue if suspected), bullous pemphigoid, severe arthralgia (post-marketing reports), and hypersensitivity reactions.
## Key Drug Interactions
* **P-glycoprotein/CYP3A4 Inducers:** Strong inducers (e.g., rifampin) may decrease linagliptin plasma concentrations. Avoid concomitant use; if unavoidable, monitor glycemic control closely.
* **Insulin/Sulfonylureas:** Concomitant use increases the risk of hypoglycemia. Consider a dose reduction of the insulin secretagogue or insulin.
## Monitoring
* **Glycemic Control:** HbA1c (typically every 3–6 months); self-monitoring of blood glucose.
* **Safety:** Monitor for symptoms of pancreatitis (persistent severe abdominal pain) and bullous pemphigoid (blistering or skin erosions).
## Clinical Pearls
* Linagliptin is unique among DPP-4 inhibitors because it does not require renal dose adjustments, making it a convenient choice for patients with chronic kidney disease.
* It does not cause weight gain and has a low risk of hypoglycemia when used as monotherapy.
* May be taken with or without food.
* If a dose is missed, take it as soon as remembered, but do not take two doses on the same day.
