# Linagliptin
## Overview
Linagliptin is a dipeptidyl peptidase-4 (DPP-4) inhibitor that increases incretin hormone activity, thereby increasing insulin release and decreasing glucagon secretion in a glucose-dependent manner. Unlike other DPP-4 inhibitors, it is primarily excreted via the biliary route rather than renal.
## Primary Indications
Type 2 diabetes mellitus (T2DM) as an adjunct to diet and exercise to improve glycemic control.
## Adult Dosing
5 mg orally once daily. May be taken with or without food.
## Pediatric Dosing
Safety and efficacy have not been established in patients younger than 18 years. Use is generally not recommended in the pediatric population.
## Dose Adjustments
* **Renal Impairment:** No dose adjustment required.
* **Hepatic Impairment:** No dose adjustment required.
## Contraindications
* Hypersensitivity to linagliptin or any component of the product.
* History of serious hypersensitivity reactions (e.g., anaphylaxis, angioedema, exfoliative skin conditions) to DPP-4 inhibitors.
## Adverse Effects
* **Common:** Nasopharyngitis, hypoglycemia (when used with secretagogues or insulin).
* **Serious:** Acute pancreatitis (rare), severe and disabling arthralgia, bullous pemphigoid, hypersensitivity reactions (angioedema, urticaria).
## Key Drug Interactions
* **Strong CYP3A4/P-gp Inducers (e.g., Rifampin):** May decrease linagliptin plasma concentrations. If co-administration is necessary, monitor glycemic control closely.
* **Secretagogues/Insulin:** Co-administration may increase the risk of hypoglycemia; consider lowering the dose of the insulin or secretagogue when initiating linagliptin.
## Monitoring
* **HbA1c:** Every 3–6 months.
* **Pancreatitis:** Monitor for signs of severe, persistent abdominal pain.
* **Hypersensitivity:** Monitor for signs of skin reactions or airway swelling.
* **Renal Function:** While dose adjustment is not required, routine monitoring remains standard for T2DM management.
## Clinical Pearls
* **Renal Sparing:** Linagliptin is the only DPP-4 inhibitor that does not require renal dose adjustments, making it convenient for patients with chronic kidney disease.
* **Pancreatitis Risk:** Always inquire about symptoms of pancreatitis (severe abdominal pain radiating to the back, nausea, vomiting) during follow-up visits.
* **Weight Neutrality:** Linagliptin is generally weight-neutral and is not associated with weight gain.
