# Tabgenixim (Cefixime)
## Overview
Tabgenixim is a brand-name formulation of **cefixime**, a third-generation cephalosporin antibiotic. It acts by inhibiting bacterial cell wall synthesis via binding to penicillin-binding proteins (PBPs). It possesses broad-spectrum activity against many Gram-negative organisms but limited activity against Gram-positive staphylococci.
## Primary Indications
* Uncomplicated urinary tract infections.
* Otitis media.
* Pharyngitis and tonsillitis.
* Acute bronchitis and acute exacerbations of chronic bronchitis.
* Uncomplicated gonorrhea (cervical/urethral).
## Adult Dosing
* **General systemic infections:** 400 mg daily, administered either as a single dose or in two divided doses (200 mg every 12 hours).
* **Uncomplicated gonorrhea:** 400 mg as a single oral dose.
## Pediatric Dosing
* **General dosing (6 months and older):** 8 mg/kg/day orally, administered as a single daily dose or in two divided doses (4 mg/kg every 12 hours).
* **Maximum pediatric dose:** 400 mg/day.

## Dose Adjustments
* **Renal Impairment:** If CrCl is 21–60 mL/min, administer 75% of the standard dose. If CrCl is ≤20 mL/min, administer 50% of the standard dose.
* **Hemodialysis/Peritoneal Dialysis:** Supplementation is generally not required; patients on dialysis should generally receive the dose adjusted for CrCl ≤20 mL/min.
## Contraindications
* Known hypersensitivity to cefixime or other cephalosporins.
* History of severe allergic reactions (anaphylaxis/urticaria) to penicillins or other beta-lactams (due to potential cross-reactivity).
## Adverse Effects
* **Gastrointestinal:** Diarrhea (frequent), abdominal pain, nausea, dyspepsia.
* **Dermatologic:** Rash, pruritus, urticaria.
* **Neurological:** Headache, dizziness.
* **Serious (Rare):** *Clostridioides difficile*-associated diarrhea, Stevens-Johnson syndrome, anaphylaxis.
## Key Drug Interactions
* **Anticoagulants:** May enhance the anticoagulant effect of warfarin (vitamin K synthesis inhibition).
* **Probenecid:** Increases serum concentrations of cefixime by reducing renal tubular secretion.
* **Nifedipine:** May increase the bioavailability of cefixime.
## Monitoring
* Monitor for signs of superinfection or *C. difficile* (persistent watery or bloody diarrhea).
* Monitor renal function in patients with pre-existing impairment.
* Assess for resolution of infection symptoms.
## Clinical Pearls
* Cefixime is generally ineffective against *Staphylococcus aureus* or *Enterococcus* species.
* Take with or without food.
* Oral suspension must be shaken well before use.
* Unlike intravenous cephalosporins, oral cephalosporins like Tabgenixim have poor CNS penetration and are not used for meningitis.
