# Linagliptin
## Overview
Linagliptin is a dipeptidyl peptidase-4 (DPP-4) inhibitor that increases incretin hormone concentrations, stimulating glucose-dependent insulin release and decreasing glucagon levels. It is unique among DPP-4 inhibitors for its primary biliary excretion and lack of significant renal clearance.
## Primary Indications
Adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus.
## Adult Dosing
* **Standard dose:** 5 mg orally once daily.
* **Maximum dose:** 5 mg daily.
## Pediatric Dosing
Safety and efficacy have not been established in pediatric patients under 18 years of age.
## Dose Adjustments
* **Renal Impairment:** No dosage adjustment necessary (regardless of stage, including end-stage renal disease).
* **Hepatic Impairment:** No dosage adjustment necessary.
## Contraindications
* Hypersensitivity to linagliptin (anaphylaxis, angioedema, exfoliative skin conditions have occurred).
* History of pancreatitis (use with caution).
* Type 1 diabetes or diabetic ketoacidosis.
## Adverse Effects
* **Common:** Nasopharyngitis, increased serum uric acid.
* **Serious:** Acute pancreatitis (including fatal outcomes), severe and disabling arthralgia, bullous pemphigoid, and hypersensitivity reactions.
## Key Drug Interactions
* **Strong CYP3A4/P-gp Inducers (e.g., Rifampin):** May decrease linagliptin systemic exposure, potentially reducing efficacy. Consider alternative glycemic agents if co-administration is necessary.
* **Insulin/Secretagogues:** May increase the risk of hypoglycemia. A dose reduction of the secretagogue or insulin may be required.
## Monitoring
* **Glycemic efficacy:** HbA1c (typically every 3 to 6 months).
* **Clinical:** Assess for signs/symptoms of pancreatitis (persistent, severe abdominal pain) and severe joint pain.
* **Dermatologic:** Monitor for blistering or skin erosions suggestive of bullous pemphigoid.
## Clinical Pearls
* **Renal sparing:** Linagliptin is the only DPP-4 inhibitor that does not require renal dose adjustments, making it a preferred choice in patients with chronic kidney disease.
* **Administration:** May be taken with or without food.
* **Not for weight loss:** Unlike GLP-1 receptor agonists, linagliptin is generally weight-neutral.
