# Linagliptin
## Overview
Linagliptin is a dipeptidyl peptidase-4 (DPP-4) inhibitor that increases incretin hormone levels, which increase insulin release and decrease glucagon levels in a glucose-dependent manner. Unlike other DPP-4 inhibitors, it is primarily excreted via the non-renal route (bile/GI tract).
## Primary Indications
Adjunct to diet and exercise to improve glycemic control in patients with type 2 diabetes mellitus.
## Adult Dosing
* **Recommended Dose:** 5 mg orally once daily.
* **Maximum Dose:** 5 mg per day.
## Pediatric Dosing
* **Safety and efficacy:** Not established in pediatric patients (under 18 years).
## Dose Adjustments
* **Hepatic Impairment:** No dose adjustment required.
* **Renal Impairment:** No dose adjustment required; safe for use in all stages of chronic kidney disease (CKD) and end-stage renal disease (ESRD).
## Contraindications
* History of hypersensitivity reaction to linagliptin (e.g., anaphylaxis, angioedema, exfoliative skin conditions).
## Adverse Effects
* **Common:** Nasopharyngitis, hypoglycemia (when used with insulin or secretagogues).
* **Serious:** Acute pancreatitis (discontinue if suspected), bullous pemphigoid, severe joint pain (arthralgia), hypersensitivity reactions.
## Key Drug Interactions
* **Strong CYP3A4/P-gp Inducers (e.g., rifampin):** May decrease linagliptin efficacy. If combined, monitor glycemic control closely.
* **Insulin/Sulfonylureas:** May increase the risk of hypoglycemia. Consider dose reduction of insulin/secretagogue if hypoglycemia occurs.
## Monitoring
* **Glycemic Control:** HbA1c (typically every 3–6 months) and fingerstick blood glucose.
* **Clinical Signs:** Monitor for new or worsening abdominal pain (pancreatitis) and skin lesions (bullous pemphigoid).
## Clinical Pearls
* **Renal Advantage:** Unique among DPP-4 inhibitors as no dose adjustment is necessary regardless of renal function.
* **Administration:** May be taken with or without food.
* **Not for T1DM:** Not indicated for type 1 diabetes or treatment of diabetic ketoacidosis.
* **Drug-Drug Interactions:** Has a lower potential for drug-drug interactions compared to saxagliptin due to its minimal metabolism by CYP enzymes.
