# Linagliptin
## Overview
Linagliptin is a dipeptidyl peptidase-4 (DPP-4) inhibitor that increases incretin hormone concentrations, stimulating glucose-dependent insulin release and decreasing glucagon secretion. Unlike most DPP-4 inhibitors, it is primarily excreted via the biliary/gastrointestinal route.
## Primary Indications
Type 2 diabetes mellitus (T2DM) to improve glycemic control as an adjunct to diet and exercise.
## Adult Dosing
* **Monotherapy/Combination:** 5 mg orally once daily.
* **Maximum Dose:** 5 mg daily.
## Pediatric Dosing
Safety and effectiveness in pediatric patients (younger than 18 years) have not been established. Use is generally not recommended.
## Dose Adjustments
* **Renal Impairment:** No dose adjustment necessary.
* **Hepatic Impairment:** No dose adjustment necessary.
* **Geriatric:** No dose adjustment necessary.
## Contraindications
* Hypersensitivity to linagliptin or any component of the formulation.
* History of angioedema or anaphylaxis with DPP-4 inhibitors.
## Adverse Effects
* **Common:** Nasopharyngitis, hypoglycemia (when used with insulin or sulfonylureas), hyperuricemia.
* **Serious:** Acute pancreatitis (rare), bullous pemphigoid (may require discontinuation), severe joint pain (arthralgia), hypersensitivity reactions (angioedema, urticaria, exfoliative skin conditions).
## Key Drug Interactions
* **Strong CYP3A4/P-gp Inducers (e.g., Rifampin):** Decreases linagliptin effectiveness; avoid co-administration.
* **Insulin/Sulfonylureas:** May increase the risk of hypoglycemia. Consider dose reduction of the secretagogue if used in combination with linagliptin.
## Monitoring
* **HbA1c:** Typically every 3 to 6 months.
* **Blood Glucose:** Self-monitoring as indicated by the patient's regimen.
* **Physical:** Monitor for signs/symptoms of pancreatitis (persistent severe abdominal pain) and bullous pemphigoid (blistering or erosions).
## Clinical Pearls
* **Excretion:** Linagliptin is unique among DPP-4 inhibitors for its predominantly non-renal (biliary) clearance, making it a preferred choice in patients with chronic kidney disease (CKD).
* **Weight:** It is considered weight-neutral.
* **Administration:** It can be taken without regard to meals.
* **Dose Omission:** If a dose is missed, take it as soon as remembered the same day; do not take two 5 mg doses on the same day.
