# Linagliptin
## Overview
Linagliptin is a dipeptidyl peptidase-4 (DPP-4) inhibitor that increases incretin hormone concentrations, stimulating glucose-dependent insulin release and decreasing glucagon levels. It is unique among DPP-4 inhibitors for its primarily biliary excretion, requiring no dose adjustments for renal or hepatic impairment.
## Primary Indications
Adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus.
## Adult Dosing
Standard dose: 5 mg orally once daily.
Maximum dose: 5 mg daily.
May be taken with or without food.
## Pediatric Dosing
Safety and efficacy have not been established in pediatric patients under 18 years of age.
## Dose Adjustments
* **Renal Impairment:** No dose adjustment required.
* **Hepatic Impairment:** No dose adjustment required.
* **Combination Therapy:** If adding to an insulin secretagogue (e.g., sulfonylurea) or insulin, consider a lower dose of the secretagogue/insulin to reduce the risk of hypoglycemia.
## Contraindications
* History of a serious hypersensitivity reaction to linagliptin (e.g., anaphylaxis, angioedema, exfoliative skin conditions).
## Adverse Effects
* **Common:** Nasopharyngitis, hypoglycemia (when used with secretagogues/insulin), hyperuricemia.
* **Serious:** Acute pancreatitis (rare), bullous pemphigoid (rare but requires discontinuation if confirmed), severe joint pain (arthralgia).
## Key Drug Interactions
* **Strong CYP3A4/P-gp Inducers (e.g., rifampin, carbamazepine, phenytoin):** May decrease linagliptin efficacy. If co-administration is necessary, monitor glycemic control closely.
* **Insulin/Secretagogues:** Increased risk of hypoglycemia.
## Monitoring
* **HbA1c:** Assess glycemic control every 3 to 6 months.
* **Pancreatitis:** Monitor for signs or symptoms (persistent, severe abdominal pain).
* **Skin:** Monitor for blistering or erosions.
## Clinical Pearls
* Linagliptin is the only DPP-4 inhibitor that does not necessitate renal dose adjustment, making it a preferred choice in patients with chronic kidney disease.
* Unlike GLP-1 receptor agonists, linagliptin is weight-neutral.
* Though clinical trial data on cardiovascular (CV) safety suggested no increased CV risk, it does not provide the CV benefit demonstrated by certain GLP-1 agonists or SGLT2 inhibitors.
