# Linagliptin
## Overview
Linagliptin is a dipeptidyl peptidase-4 (DPP-4) inhibitor that increases incretin hormone concentrations (GLP-1 and GIP), which enhance insulin secretion and decrease glucagon levels in a glucose-dependent manner.
## Primary Indications
Type 2 diabetes mellitus (T2DM) as an adjunct to diet and exercise to improve glycemic control.
## Adult Dosing
* **Standard Dose:** 5 mg orally once daily.
* **Maximum Dose:** 5 mg daily.
## Pediatric Dosing
Safety and efficacy have not been established in pediatric patients (<18 years). Use is generally not recommended.
## Dose Adjustments
* **Renal Impairment:** No dose adjustment required; safe for use in all stages of chronic kidney disease (CKD) and end-stage renal disease (ESRD).
* **Hepatic Impairment:** No dose adjustment required.
## Contraindications
* History of serious hypersensitivity reactions to linagliptin (e.g., anaphylaxis, angioedema, exfoliative skin conditions).
## Adverse Effects
* **Common:** Nasopharyngitis, diarrhea, arthralgia.
* **Serious:** Hypersensitivity reactions (urticaria, angioedema), bullous pemphigoid, severe joint pain, and acute pancreatitis.
## Key Drug Interactions
* **Strong CYP3A4/P-gp Inducers (e.g., Rifampin):** May decrease linagliptin efficacy. If possible, consider alternative antidiabetic therapies if co-administration is necessary.
* **Insulin/Secretagogues:** Increased risk of hypoglycemia when used concomitantly; dose reduction of the secretagogue or insulin may be required.
## Monitoring
* **Glycemic control:** HbA1c (typically every 3–6 months), fasting plasma glucose.
* **Clinical:** Monitor for signs/symptoms of pancreatitis (persistent, severe abdominal pain) and bullous pemphigoid (blistering or erosions).
## Clinical Pearls
* **Renal Advantage:** Unlike other DPP-4 inhibitors (e.g., saxagliptin, sitagliptin), linagliptin does not require renal dose adjustments as it is primarily excreted via the hepatobiliary system.
* **Administration:** May be taken with or without food.
* **Pancreatitis:** Discontinue immediately if pancreatitis is suspected.
* **Weight:** Linagliptin is weight-neutral.
