# Linagliptin
## Overview
Linagliptin is a dipeptidyl peptidase-4 (DPP-4) inhibitor that increases incretin hormone concentrations (GLP-1 and GIP), resulting in glucose-dependent insulin secretion and decreased glucagon release. It is unique among DPP-4 inhibitors due to its primarily biliary/gastrointestinal excretion rather than renal.
## Primary Indications
Adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus.
## Adult Dosing
Standard dose: 5 mg orally once daily.
Maximum dose: 5 mg daily.
## Pediatric Dosing
Safety and efficacy have not been established in patients younger than 18 years. Use is generally not recommended in pediatric populations.
## Dose Adjustments
* **Renal Impairment:** No dose adjustment required (regardless of eGFR).
* **Hepatic Impairment:** No dose adjustment required.
* **Strong CYP3A4/P-gp Inducers:** If co-administration with rifampin is necessary, clinical efficacy may be reduced. Consider intensifying glycemic monitoring.
## Contraindications
* Hypersensitivity to linagliptin (e.g., anaphylaxis, angioedema, exfoliative skin conditions).
* History of pancreatitis (use with caution).
## Adverse Effects
* **Common:** Nasopharyngitis, hypoglycemia (when used with insulin or sulfonylureas).
* **Serious:** Acute pancreatitis, severe/disabling arthralgia, bullous pemphigoid, hypersensitivity reactions, and heart failure (reported with some DPP-4 inhibitors).
## Key Drug Interactions
* **CYP3A4/P-gp Inducers (e.g., Rifampin):** May decrease linagliptin plasma concentrations, potentially reducing therapeutic effect.
* **Sulfonylureas/Insulin:** Increased risk of hypoglycemia; a lower dose of the insulin or secretagogue may be required.
## Monitoring
* **HbA1c:** Typically every 3 to 6 months.
* **Blood Glucose:** Periodic fingerstick monitoring, especially if adding to insulin or secretagogues.
* **Pancreatitis:** Monitor for unexplained, severe abdominal pain.
* **Skin:** Monitor for blistering or erosions suggestive of bullous pemphigoid.
## Clinical Pearls
* **Renal Benefit:** Linagliptin is the preferred DPP-4 inhibitor for patients with end-stage renal disease (ESRD) or severe renal impairment because it does not require dosage titration.
* **Administration:** May be taken with or without food.
* **Weight Neutrality:** Linagliptin is generally weight-neutral.
* **Heart Failure:** While linagliptin did not show an increased risk of heart failure hospitalizations in the CARMELINA trial, caution is advised in patients with pre-existing heart failure.
