# Genixim (Cefixime)
## Overview
Genixim is a third-generation oral cephalosporin antibiotic. It possesses broad-spectrum activity against Gram-negative organisms (e.g., *Haemophilus influenzae*, *Moraxella catarrhalis*, *Neisseria gonorrhoeae*) and limited activity against Gram-positive organisms.
## Primary Indications
* Uncomplicated urinary tract infections (UTIs)
* Otitis media
* Pharyngitis and tonsillitis
* Acute bronchitis and acute exacerbations of chronic bronchitis
* Uncomplicated gonorrhea (cervical, urethral, or rectal)
## Adult Dosing
* **General infections:** 400 mg daily, administered either as a single dose or divided into 200 mg every 12 hours.
* **Uncomplicated gonorrhea:** 800 mg (two 400 mg tablets) as a single oral dose.
## Pediatric Dosing
* **Children (>6 months or weight >50 kg):** 8 mg/kg/day, administered either as a single daily dose or divided into 4 mg/kg every 12 hours.
* **Children weighting >50 kg:** Use adult dosing.
## Dose Adjustments
* **Renal Impairment:** If CrCl <60 mL/min, dosage adjustments may be required (often 75% of the standard dose if CrCl is 21–60 mL/min; 50% of the standard dose if CrCl <20 mL/min). Clinical judgment and current renal function monitoring are essential.
## Contraindications
* Known hypersensitivity to cefixime or other cephalosporins (e.g., cephalexin, ceftriaxone).
* History of severe anaphylactic reactions to penicillins or other beta-lactams.
## Adverse Effects
* **Common:** Diarrhea, abdominal pain, nausea, dyspepsia, flatulence.
* **Severe:** *Clostridioides difficile*-associated diarrhea (CDAD), hypersensitivity reactions (rash, urticaria, anaphylaxis), and rare cases of Stevens-Johnson syndrome.
## Key Drug Interactions
* **Warfarin/Anticoagulants:** May increase the anticoagulant effect (monitor INR).
* **Probenecid:** Increases serum concentrations of cefixime by inhibiting renal excretion.
## Monitoring
* Monitor for signs of superinfection (*C. difficile* diarrhea).
* Assess for resolution of infection symptoms.
* Monitor renal function in patients with pre-existing impairment or elderly patients.
## Clinical Pearls
* **Absorption:** Oral bioavailability is not clinically affected by the presence of food.
* **Spectrum:** Cefixime has poor activity against *Staphylococcus aureus*. It should not be used as empiric therapy for infections likely caused by *S. aureus* or enterococci.
* **Compliance:** The once-daily dosing regimen significantly improves adherence compared to multiple daily doses.
