# Tab amiodarone
## Overview
- **Classification**: Class III antiarrhythmic agent.
- **Mechanism**: Prolongs cardiac action potential and refractory period, primarily by blocking potassium channels. Also has alpha/beta blocking, calcium channel blocking, and sodium channel blocking properties.
## Primary Indications
1. **Ventricular Arrhythmias**: Treatment and prophylaxis of recurrent ventricular fibrillation (VF) and hemodynamically unstable ventricular tachycardia (VT).
2. **Supraventricular Arrhythmias**: Rate control and rhythm conversion in atrial fibrillation (AFib) or atrial flutter (AFlutter), especially with structural heart disease or refractory to other agents.
## Adult Dosing
### Standard Dosing
**Life-threatening Ventricular Arrhythmias (e.g., recurrent VF/unstable VT)**
- **Loading Dose**: **800-1600 mg/day**.
- **Frequency**: Once daily or divided **BID**.
- **Route**: Oral.
- **Duration**: **1-3 weeks** or until initial therapeutic response.
- **Maintenance Dose**: **400 mg/day**.
- **Frequency**: Once daily.
**Supraventricular Arrhythmias (e.g., AFib)**
- **Loading Dose**: **600-800 mg/day**.
- **Frequency**: Once daily or divided **BID**.
- **Route**: Oral.
- **Duration**: **1-2 weeks**.
- **Maintenance Dose**: **100-400 mg/day**.
- **Frequency**: Once daily.
- **Special Considerations**: Use the lowest effective dose.
### Dose Adjustments
- **Renal Impairment**: No specific dose adjustment is generally needed. Monitor for adverse effects.
- **Hepatic Impairment**: Use with caution. Consider lower starting doses and slower titration. Monitor LFTs closely; avoid in severe hepatic dysfunction.
- **Elderly Patients**: May be more susceptible to adverse effects. Start with lower doses and titrate slowly. Monitor thyroid, pulmonary, and liver function.
## Pediatric Dosing

### Neonates (0-28 days)
- **Indication**: Refractory supraventricular or ventricular arrhythmias.
- **Dose**: **10-15 mg/kg/day** (loading), then **5-10 mg/kg/day** (maintenance).
- **Frequency**: Once daily or divided **BID**.
- **Maximum**: **15 mg/kg/day** (loading), **300 mg/day** (overall).
- **Special Notes**: Highly variable pharmacokinetics. Requires close ECG and plasma level monitoring. Oral solutions (compounded) are often used.
### Infants (1-12 months)
- **Indication**: Refractory supraventricular or ventricular arrhythmias.
- **Dose**: **10-15 mg/kg/day** (loading) for **7-10 days**.
- **Frequency**: Once daily or divided **BID**.
- **Maximum**: **300 mg/day** during loading.
- **Maintenance**: **5-10 mg/kg/day**, adjusted to response.
- **Special Notes**: Monitor for thyroid, liver, and pulmonary toxicity. Use lowest effective dose.
### Children (1-12 years)
- **Indication**: Refractory supraventricular or ventricular arrhythmias.
- **Dose**: **10-15 mg/kg/day** (loading) for **7-10 days**.
- **Frequency**: Once daily or divided **BID**.
- **Maximum**: **300 mg/day** during loading.
- **Maintenance**: **5-10 mg/kg/day**, adjusted to response.

### Adolescents (13-18 years)
- **Dose**: Generally follows adult dosing guidelines.

- **Maximum**: **1600 mg/day** for loading, **400 mg/day** for maintenance.

## Safety Information
### Contraindications
- **Absolute**: Severe sinus node dysfunction (e.g., sinus bradycardia, SA block) without a functional pacemaker.
- **Absolute**: Second or third-degree AV block without a functional pacemaker.
- **Absolute**: Hypersensitivity to amiodarone or iodine.
- **Absolute**: Cardiogenic shock.
### Common Adverse Effects
- **Very Common (>10%)**: Corneal microdeposits (usually asymptomatic), GI upset (nausea, vomiting, constipation), photosensitivity.
- **Common (1-10%)**: Thyroid dysfunction (hypo/hyperthyroidism), liver enzyme elevation, peripheral neuropathy, tremor, dizziness, fatigue, skin discoloration (blue-gray).
- **Serious but Rare**: Pulmonary fibrosis, severe hepatotoxicity, exacerbation of arrhythmia (proarrhythmia), optic neuropathy/neuritis, Torsades de Pointes.
### Key Drug Interactions
- **Warfarin**: **Increased INR/bleeding risk**. Reduce warfarin dose by **30-50%** when starting amiodarone. Monitor INR closely.
- **Digoxin**: **Increased digoxin levels**. Reduce digoxin dose by **50%**. Monitor digoxin levels.
- **Statins (Simvastatin, Lovastatin)**: **Increased statin levels/rhabdomyolysis risk**. Limit simvastatin to **20 mg/day** or use alternative statin (e.g., rosuvastatin, pravastatin).
- **QT-prolonging drugs**: **Increased risk of Torsades de Pointes**. Avoid co-administration if possible (e.g., fluoroquinolones, macrolides, TCAs, some antipsychotics).
- **CYP3A4 inhibitors (e.g., grapefruit juice, azole antifungals, macrolides)**: **Increased amiodarone levels**. Monitor for toxicity.
- **CYP3A4 inducers (e.g., rifampin, St. John's wort)**: **Decreased amiodarone levels**. May reduce efficacy.
## Monitoring & Follow-up
- **Before Treatment**: Baseline ECG, chest X-ray, pulmonary function tests (PFTs), thyroid function tests (TSH, free T4), liver function tests (ALT, AST), electrolytes (K+, Mg2+).
- **During Treatment**:
- **LFTs & TSH**: Every **3-6 months** or as clinically indicated.
- **ECG**: Periodically to assess rhythm control and QTc interval.
- **Ophthalmologic exam**: Annually or if visual changes occur.
- **Pulmonary assessment**: If symptoms (dyspnea, cough) arise; consider chest X-ray/PFTs.
- **Clinical Signs**: Watch for dyspnea, dry cough, jaundice, unexplained fatigue, vision changes, new bradycardia, or signs of worsening arrhythmia.
## Clinical Pearls
- 💡 **Long Half-Life**: Amiodarone has an extremely long half-life (**up to 58 days**). Effects and side effects can persist for months after discontinuation.
- 💡 **Oral Absorption**: Highly variable. Take with food to improve absorption and reduce GI upset.
- 💡 **Photosensitivity**: Counsel patients to use sunscreen and protective clothing. Blue-gray skin discoloration can occur with long-term use.
- 💡 **Prophylactic Pacing**: Essential if patient has pre-existing bradyarrhythmias or conduction blocks before initiation.
- 💡 **Pill Burden**: The high loading doses can be challenging for adherence. Counsel patients on the importance of consistent dosing.
