# Shampoo ketoconazole
## Overview
- **Classification**: Antifungal, Azole (Imidazole derivative)
- **Mechanism**: Inhibits cytochrome P450-dependent C-14 alpha-demethylation of lanosterol, a precursor to ergosterol. This disrupts fungal cell membrane synthesis, leading to increased permeability and cell death.
## Primary Indications
1. **Seborrheic Dermatitis** - Management of flaky, itchy scalp/skin (dandruff).
2. **Tinea Versicolor** - Treatment of a common superficial fungal infection of the skin.
## Adult Dosing
### Standard Dosing
**Seborrheic Dermatitis (Dandruff)**
- **Dose**: Apply **5 mL** (or enough to lather) of **2% ketoconazole shampoo** to affected areas.
- **Frequency**: Twice weekly for 2-4 weeks.
- **Route**: Topical to scalp/affected skin.
- **Duration**: Followed by once every 1-2 weeks for maintenance if needed.
**Tinea Versicolor**

- **Elderly Patients**: No specific adjustments needed. Use with caution for sensitive skin.
## Pediatric Dosing
### Neonates (0-28 days)
- **Dose**: **Not recommended** due to lack of safety data.
- **Special Notes**: Consult a dermatologist or pediatrician. Avoid use unless specifically directed.
### Infants (1-12 months)
- **Dose**: **Not recommended** due to limited safety data.
- **Special Notes**: Use only under direct medical supervision for specific indications, as absorption could be higher through infant skin.
### Children (1-12 years)
- **Dose**: **Off-label use**. If prescribed, often **2% ketoconazole shampoo**.
- **Frequency**: Typically **twice weekly** for 2-4 weeks for seborrheic dermatitis.
- **Maximum**: Use smallest amount for lather; limit contact time to **3-5 minutes**.
- **Special Notes**: Use with extreme caution. Avoid eye contact. Supervise application to prevent accidental ingestion.
### Adolescents (13-18 years)
- **Dose**: Similar to **adult dosing** for seborrheic dermatitis or tinea versicolor.
- **Maximum**: Follow adult duration and frequency guidelines.
## Safety Information
### Contraindications
- **Absolute**: Hypersensitivity to ketoconazole or any component of the shampoo.
### Common Adverse Effects
- **Common (1-10%)**: Itching, stinging, irritation, dry skin at application site.
- **Common (1-10%)**: Abnormal hair texture, hair discoloration (especially in chemically damaged or gray hair).
- **Less Common**: Burning sensation, rash, oily/dry hair.
- **Serious but Rare**: Severe allergic reactions (e.g., angioedema).
### Key Drug Interactions
- **Topical application**: Systemic absorption is negligible.
- **Systemic Interactions**: Generally **not clinically significant** for shampoo formulation.
- **Other Topical Agents**: Avoid concurrent use of other medicated topical products on the same area unless directed by a clinician, to prevent irritation.
## Monitoring & Follow-up
- **Before Treatment**: Assess skin for integrity; identify areas to treat.
- **During Treatment**: Monitor for signs of local irritation, redness, or increased itching.
- **Clinical Signs**: Assess for resolution of scaling, itching, or lesions. If no improvement after 2-4 weeks, re-evaluate diagnosis.
## Clinical Pearls
- 💡 **Application**: Lather shampoo thoroughly, leave on for **3-5 minutes** for optimal efficacy, then rinse completely.
- 💡 **Eye Protection**: **Avoid contact with eyes**. If contact occurs, rinse immediately and thoroughly with water.
- 💡 **Hair Considerations**: May alter texture or color of permed, colored, or gray hair.
- 💡 **Maintenance**: For seborrheic dermatitis, once weekly or bi-weekly use can help prevent recurrence.
- 💡 **Tinea Versicolor**: Apply to a wide area beyond visible lesions. May require re-treatment for recurrence.
