# Secnidazole
## Overview
- **Classification**: Nitroimidazole antimicrobial
- **Mechanism**: Activated metabolite forms reactive intermediates that bind to and damage bacterial DNA, inhibiting nucleic acid synthesis and leading to cell death.
## Primary Indications
1. **Bacterial Vaginosis (BV)** - Single-dose oral treatment for symptomatic BV.
2. **Trichomoniasis** - Single-dose oral treatment for Trichomonas vaginalis infection.
## Adult Dosing

## Pediatric Dosing
*Note: Secnidazole for Bacterial Vaginosis and Trichomoniasis is not indicated for pediatric patients below 18 years of age due to lack of established safety and efficacy.*
*However, secnidazole is used off-label in pediatrics for other protozoal infections like amebiasis or giardiasis in some regions.*
### Neonates (0-28 days)
- **Not Recommended**: Safety and efficacy not established.
### Infants (1-12 months)
- **Not Recommended**: Safety and efficacy not established for primary indications.
### Children (1-12 years)
- **Amebiasis/Giardiasis (Off-label use)**
- **Dose**: **30 mg/kg**

- **Maximum**: **2 g**
- **Special Notes**: Use only for indicated parasitic infections; consult infectious disease specialist. Not for BV/Trichomoniasis.
### Adolescents (13-18 years)
- **Bacterial Vaginosis/Trichomoniasis**: Limited data.
- **Dose**: If prescribed, may consider adult dose of **2 g**
- **Maximum**: **2 g**
- **Special Notes**: Only use if clear indication and potential benefits outweigh risks.
## Safety Information
### Contraindications
- **Absolute**: Hypersensitivity to secnidazole or other nitroimidazole derivatives.
- **Absolute**: Pregnancy (particularly 1st trimester). Avoid use in all trimesters unless essential.
- **Absolute**: Breastfeeding (discontinue for **96 hours** after dose).
### Common Adverse Effects
- **Very Common (>10%)**: Vulvovaginal candidiasis, headache, nausea, dysgeusia (metallic taste).
- **Common (1-10%)**: Abdominal pain, diarrhea, vomiting, dizziness, pruritus.
- **Serious but Rare**: Leukopenia, neutropenia (transient), peripheral neuropathy.
### Key Drug Interactions
- **Alcohol**: Disulfiram-like reaction (flushing, palpitations, nausea, vomiting, abdominal cramps). **AVOID** alcohol during treatment and for **48 hours** after.
- **Warfarin**: May potentiate anticoagulant effect. Monitor INR/PT closely.
- **Lithium**: May increase lithium levels. Monitor lithium concentrations.
- **Disulfiram**: Concomitant use may lead to psychotic reactions. Avoid.
## Monitoring & Follow-up
- **Before Treatment**: Pregnancy test (for women of childbearing potential).
- **During Treatment**: Assess for resolution of symptoms.
- **Clinical Signs**: Monitor for severe GI upset, neurological symptoms (dizziness, paresthesias), or signs of allergic reaction.
## Clinical Pearls
- 💡 **Single-dose convenience**: Enhances patient adherence significantly.
- 💡 **Granule administration**: Mix entire contents of packet with soft food (e.g., applesauce, yogurt, pudding); do NOT dissolve in liquid. Consume immediately.
- 💡 **Alcohol avoidance**: Crucial patient counseling point due to disulfiram-like reaction.
- 💡 **Taste masking**: Can have a bitter or metallic taste; mixing with strong-flavored soft foods helps.
