# Raniped
## Overview
- **Classification**: Proton Pump Inhibitor (PPI)
- **Mechanism**: Irreversibly blocks the H+/K+-ATPase proton pump in gastric parietal cells, reducing gastric acid secretion.
## Primary Indications
1. **Gastroesophageal Reflux Disease (GERD)** - Symptomatic relief and healing of esophagitis.
2. **Erosive Esophagitis** - Treatment and maintenance of healing.
3. **Peptic Ulcer Disease (PUD)** - Treatment of gastric and duodenal ulcers.
4. **_Helicobacter pylori_ Eradication** - Part of a multi-drug regimen.
## Adult Dosing

**Peptic Ulcer Disease (PUD)**
- **Dose**: **20 mg**
- **Frequency**: Once daily
- **Route**: Oral
- **Duration**: 4 weeks for duodenal, 8 weeks for gastric ulcer.
### Dose Adjustments
- **Renal Impairment**: No dose adjustment generally required. Use with caution in severe renal dysfunction.
- **Hepatic Impairment**: For severe hepatic impairment (Child-Pugh C), consider dose reduction to **20 mg** daily.
- **Elderly Patients**: No specific dose adjustment needed. Monitor for adverse effects.
## Pediatric Dosing
### Neonates (0-28 days)
- **Indication**: Severe GERD/Esophagitis resistant to other therapies.
- **Dose**: **0.7 mg/kg**
- **Frequency**: Once daily
- **Maximum**: **10 mg/day**
- **Special Notes**: Not routinely recommended. Use liquid formulation if available. Monitor closely for adverse effects.
### Infants (1-12 months)
- **Indication**: Symptomatic GERD, erosive esophagitis.
- **Dose**: **0.7-1 mg/kg**
- **Frequency**: Once daily
- **Maximum**: **20 mg/day**
- **Special Notes**: Administer 30-60 min before first feed.
### Children (1-12 years)

- **Dose**:
* < 20 kg: **10 mg** once daily
* ≥ 20 kg: **20 mg** once daily
- **Frequency**: Once daily
- **Maximum**: **40 mg/day** (for severe cases or _H. pylori_)
### Adolescents (13-18 years)

- **Maximum**: **40 mg/day**
## Safety Information
### Contraindications
- **Absolute**: Hypersensitivity to Raniped or any component.
- **Absolute**: Concomitant use with rilpivirine-containing products.
### Common Adverse Effects
- **Very Common (>10%)**: Headache
- **Common (1-10%)**: Nausea, diarrhea, abdominal pain, flatulence, constipation.
- **Serious but Rare**: _Clostridioides difficile_ infection (CDI), bone fractures (long-term use), acute interstitial nephritis, subacute cutaneous lupus erythematosus (SCLE), vitamin B12 deficiency (long-term).
### Key Drug Interactions
- **Clopidogrel**: May reduce antiplatelet effect. Consider alternative antiplatelet or PPI.
- **Methotrexate**: May increase methotrexate levels, leading to toxicity. Monitor and adjust methotrexate dose.
- **Iron Salts/Calcium Carbonate**: May reduce absorption due to increased gastric pH. Administer separately if possible.
- **Antifungals (e.g., Ketoconazole)**: May reduce absorption. Avoid co-administration if possible.
## Monitoring & Follow-up
- **Before Treatment**: Rule out malignancy if alarming symptoms present.
- **During Treatment**: Symptom resolution, tolerance.
- **Long-term (>1 year)**: Monitor for Vitamin B12 deficiency, magnesium levels (especially with diuretics), and bone density.
- **Clinical Signs**: Watch for unexplained weight loss, dysphagia, hematemesis, melena (red flags requiring endoscopy).
## Clinical Pearls
- 💡 **Timing**: Administer Raniped **30-60 minutes before the first meal** of the day for optimal effect.
- 💡 **Long-term Use**: Re-evaluate the need for continued PPI therapy periodically; chronic use linked to several risks.
- 💡 **Administration**: Do not crush or chew capsules/tablets. For patients unable to swallow, use oral suspension or open capsule and mix contents with applesauce (do not chew pellets).
