# Phloroglucinol
## Overview
- **Classification**: Antispasmodic
- **Mechanism**: Phloroglucinol is a non-atropinic antispasmodic. It acts directly on smooth muscle cells, reducing their tonicity and spasms in the gastrointestinal, biliary, and genitourinary tracts.
## Primary Indications
1. **Spasmodic Pain**: Relief of functional disorders of the GI tract (e.g., IBS), biliary system, or urinary tract (e.g., renal colic).
2. **Female Genital Tract Spasms**: Management of dysmenorrhea and as an adjuvant in labor to facilitate cervical dilation.
## Adult Dosing
### Standard Dosing
**Spasmodic Pain (Oral)**
- **Dose**: **80 mg**
- **Frequency**: **Take 2 tablets (160 mg)** at onset of pain, then **1-2 tablets (80-160 mg)** as needed.
- **Route**: Oral
- **Maximum**: Up to **6 tablets (480 mg)** per day.
**Acute Spasms (Parenteral)**
- **Dose**: **40 mg**
- **Frequency**: Administer slowly; may repeat with **40 mg** after 30-60 min if needed.
- **Route**: IV or IM
- **Maximum**: Up to **120 mg** per 24 hours.
### Dose Adjustments
- **Renal Impairment**: No specific dose adjustments provided; use with caution.
- **Hepatic Impairment**: No specific dose adjustments provided; use with caution.
- **Elderly Patients**: No specific dose adjustments; use usual adult dose with caution.
## Pediatric Dosing
### Neonates (0-28 days)
- **Not Recommended**: Safety and efficacy are not established.
- **Special Notes**: Avoid use; consider alternative treatments.
### Infants (1-12 months)
- **Not Recommended**: Safety and efficacy are not established.
- **Special Notes**: Avoid use; consider alternative treatments.
### Children (1-12 years)
**Spasmodic Pain**
- **Age Group**: Children **> 6 years**
- **Dose**: **80 mg**
- **Frequency**: **Twice daily**, up to **3 times daily** if needed.
- **Route**: Oral
- **Maximum**: **240 mg/day**.
### Adolescents (13-18 years)
**Spasmodic Pain**
- **Dose**: Adult dosing: **80 mg**
- **Frequency**: **2-3 times daily** as needed.
- **Route**: Oral
- **Maximum**: **480 mg/day**.
## Safety Information
### Contraindications
- **Absolute**: Hypersensitivity to phloroglucinol.
- **Absolute**: Mechanical obstruction of the gastrointestinal or urinary tract.
- **Absolute**: Paralytic ileus.
- **Absolute**: Megacolon.
### Common Adverse Effects
- **Rare (<1%)**: Skin rash, pruritus, urticaria, dizziness.
- **Serious but Rare**: Anaphylactic shock (especially with IV administration), angioedema, hypotension (with rapid IV injection).
### Key Drug Interactions
- **Analgesics/Opioids**: May potentiate the antispasmodic effect.
- **Other Antispasmodics**: May have synergistic effects; monitor for increased side effects.
## Monitoring & Follow-up
- **Before Treatment**: Exclude mechanical obstruction, especially in acute abdominal pain.
- **During Treatment**: Monitor for symptom relief and resolution of spasms.
- **Clinical Signs**: Watch for signs of allergic reaction (rash, itching, swelling) or hypotension (with IV).
## Clinical Pearls
- 💡 **Tip 1**: Phloroglucinol is a direct smooth muscle relaxant, avoiding anticholinergic effects.
- 💡 **Tip 2**: It is often preferred when anticholinergics are contraindicated (e.g., in glaucoma, prostatic hypertrophy).
- 💡 **Tip 3**: Parenteral forms provide rapid relief for acute, severe spasmodic pain.
- 💡 **Tip 4**: Counsel patients that dizziness may occur; advise caution with driving or operating machinery.
