# Olopatadine ## Overview - Second-generation antihistamine (H1-receptor antagonist) and mast cell stabilizer. - Available as ophthalmic solution (0.1%, 0.2%, 0.7%) and nasal spray (0.6%, 0.665%). ## Primary Indications - Allergic conjunctivitis (itch, redness, tearing) - Perennial allergic rhinitis (nasal spray) - Allergic rhinitis symptoms (nasal spray) ## Adult Dosing **Ophthalmic:** - 0.1% solution: 1 drop in affected eye(s) twice daily (8 hours apart) - 0.2% solution: 1 drop in affected eye(s) once daily - 0.7% solution: 1 drop in affected eye(s) once daily (for ocular itching) **Nasal spray:** - 0.6% or 0.665%: 2 sprays per nostril twice daily - Maximum: 2 sprays per nostril twice daily ## Pediatric Dosing **Ophthalmic:** - ≥2 years: Same as adult dosing (all strengths) **Nasal spray:** - ≥6 years: 2 sprays per nostril twice daily (0.6% or 0.665%) - <6 years: Safety and efficacy not established ## Dose Adjustments **Renal impairment:** No specific adjustment required. **Hepatic impairment:** No specific data; use with caution. ## Contraindications - Hypersensitivity to olopatadine or any component - Contact lens wearers: remove lenses before instillation; wait 10 minutes before reinserting (preservative may be absorbed by soft lenses) ## Adverse Effects - **Ophthalmic:** Headache, bitter taste, blurred vision, eye pain, dry eye - **Nasal:** Bitter taste, epistaxis, headache, nasal discomfort, sedation (rare) - Common: mild, transient stinging or burning upon instillation ## Key Drug Interactions - No significant systemic drug interactions due to minimal absorption - Additive CNS depression if combined with alcohol or other CNS depressants (rare at ophthalmic doses) ## Monitoring - Symptom relief (itching, redness, rhinorrhea, nasal congestion) - Ophthalmic: visual acuity if blurred vision persists - Nasal: monitor for nasal irritation or bleeding ## Clinical Pearls - Onset of action: within minutes for itching; full effect in 1–2 weeks for nasal spray - Higher concentration (0.7%) may be used for faster itch relief in allergic conjunctivitis - Nasal spray contains benzalkonium chloride – do not use with contact lenses (ophthalmic form) - Not effective for immediate allergic emergencies (e.g., anaphylaxis) --- *Educational disclaimer: This information is for educational purposes and does not replace professional clinical judgment.
