# Octreotide ## Overview Octreotide is a synthetic somatostatin analog that inhibits growth hormone, insulin, glucagon, and gastrointestinal peptide secretion. Available as immediate-release (SC/IV) and long-acting release (LAR, IM) formulations. ## Primary Indications - Acromegaly (adjunctive or when surgery/radiation not appropriate) - Carcinoid syndrome (control of flushing and diarrhea) - VIPomas (control of profuse watery diarrhea) - Variceal bleeding (off-label but standard; short-acting IV only) ## Adult Dosing **Acromegaly** - Initial: 50 mcg SC TID; titrate based on GH/IGF-1 levels and tolerability (typical 100–500 mcg TID). - Maintenance LAR: 20 mg IM every 4 weeks; adjust to 10–40 mg monthly. **Carcinoid syndrome / VIPoma** - Short-acting: 100–600 mcg/day SC in 2–4 divided doses (start 50 mcg BID-TID). - LAR: 20 mg IM monthly; may increase to 30
