# norfloxacin
## Overview
- **Classification**: Fluoroquinolone Antibiotic
- **Mechanism**: Inhibits bacterial DNA gyrase and topoisomerase IV, essential enzymes for DNA replication, transcription, repair, and recombination, leading to bacterial cell death.
## Primary Indications
1. **Complicated and Uncomplicated Urinary Tract Infections (UTIs)** - Including cystitis, pyelitis, and pyelonephritis.
2. **Prostatitis** - Chronic bacterial prostatitis.
3. **Gastroenteritis (Traveler's Diarrhea)** - Caused by susceptible bacteria.
## Adult Dosing
### Standard Dosing
**Uncomplicated Urinary Tract Infections**
- **Dose**: **400 mg**
- **Frequency**: Every 12 hours (BID)
- **Route**: Oral
- **Duration**: 3 to 10 days, depending on severity and pathogen. For acute cystitis, 3 days.
**Complicated Urinary Tract Infections**
- **Dose**: **400 mg**
- **Frequency**: Every 12 hours (BID)
- **Route**: Oral
- **Duration**: 10 to 21 days
**Chronic Bacterial Prostatitis**
- **Dose**: **400 mg**

### Dose Adjustments
- **Renal Impairment**:
- **CrCl >30 mL/min**: No dose adjustment needed.
- **CrCl ≤30 mL/min**: **400 mg** once daily.
- **Hepatic Impairment**: No specific dose adjustment guidelines; use with caution.
- **Elderly Patients**: No specific dose adjustment based on age alone, but monitor renal function.
## Pediatric Dosing
**Norfloxacin is generally NOT recommended for patients under 18 years of age due to the risk of arthropathy (joint damage) and other serious adverse effects. Safety and efficacy have not been established in pediatric patients.**

## Safety Information
### Contraindications
- **Absolute**: Hypersensitivity to norfloxacin or other fluoroquinolones.
- **Absolute**: History of tendon rupture or tendinitis associated with fluoroquinolone use.
- **Absolute**: Pediatric patients (generally <18 years of age).
- **Absolute**: Myasthenia gravis (may exacerbate muscle weakness).
### Common Adverse Effects
- **Common (1-10%)**: Nausea, headache, dizziness, abdominal pain, asthenia.
- **Less Common (<1%)**: Rash, diarrhea, dyspepsia, flatulence, constipation, visual disturbances.
- **Serious but Rare**:
- **Tendonitis and Tendon Rupture**: Especially Achilles tendon; risk increased with age, corticosteroids, and renal failure.
- **Peripheral Neuropathy**: May be irreversible.
- **Central Nervous System Effects**: Seizures, psychosis, suicidal thoughts, tremors.
- **QT Prolongation and Arrhythmias**: Torsades de Pointes.
- **Clostridioides difficile-associated diarrhea (CDAD)**: Can range from mild to life-threatening.
- **Hypoglycemia/Hyperglycemia**: Especially in diabetic patients.
- **Photosensitivity reactions**.
### Key Drug Interactions
- **Multivalent Cations (Antacids, Iron, Zinc, Calcium, Dairy)**: Form insoluble chelates, significantly reducing norfloxacin absorption.
- **Management**: Administer norfloxacin **2 hours before or 2 hours after** these agents.
- **Theophylline**: Norfloxacin inhibits the metabolism of theophylline, increasing its plasma levels and toxicity risk.
- **Management**: Monitor theophylline levels closely; adjust theophylline dose as needed.
- **Warfarin**: May enhance the anticoagulant effect of warfarin, increasing bleeding risk.
- **Management**: Monitor INR closely; adjust warfarin dose as needed.
- **NSAIDs**: Concomitant use with fluoroquinolones may increase the risk of CNS stimulation and convulsive seizures.
- **Management**: Use with caution; monitor for CNS symptoms.
- **Corticosteroids**: Concomitant use increases the risk of tendon rupture.
- **Management**: Avoid concomitant use if possible, especially in elderly patients.
## Monitoring & Follow-up
- **Before Treatment**:
- Assess renal function (CrCl).
- Review patient history for contraindications (tendon issues, myasthenia gravis).
- **During Treatment**:
- Monitor for signs of tendon pain, inflammation, or rupture.
- Monitor for neurological or psychiatric symptoms.
- Monitor blood glucose in diabetic patients.
- Monitor INR if co-administered with warfarin.
- **Clinical Signs**:
- Discontinue immediately if tendon pain, swelling, inflammation, or rupture occurs.
- Discontinue if new or worsening peripheral neuropathy symptoms develop.
- Discontinue if CNS symptoms (seizures, psychosis) occur.
- Monitor for signs of C. difficile infection (persistent diarrhea).
## Clinical Pearls
- 💡 **Take on Empty Stomach**: Administer norfloxacin **at least 1 hour before or 2 hours after meals** for optimal absorption.
- 💡 **Separate from Cations**: Advise patients to separate norfloxacin administration from antacids, iron, zinc, multivitamins, and dairy products.
- 💡 **Hydration**: Encourage adequate hydration to prevent crystalluria.
- 💡 **Photosensitivity**: Counsel patients to avoid excessive sun exposure and use protective clothing/sunscreen.
- 💡 **Black Box Warning**: Emphasize the potential for serious adverse effects like tendon rupture, peripheral neuropathy, and CNS effects. Instruct patients to report any unusual symptoms immediately.
