# Montelukast/Levocetirizine
## Overview
Montelukast/levocetirizine is a fixed-dose combination product used for the management of allergic rhinitis and chronic idiopathic urticaria. Montelukast is a leukotriene receptor antagonist, and levocetirizine is a second-generation H1 antihistamine.
## Primary Indications
* Allergic Rhinitis (seasonal and perennial)
* Chronic Idiopathic Urticaria
## Adult Dosing
* The typical dose is **montelukast 10 mg / levocetirizine 5 mg** taken orally once daily.
## Pediatric Dosing
* **Ages 6 to 11 years:** montelukast 5 mg / levocetirizine 2.5 mg orally once daily.
* **Ages less than 6 years:** This combination product is generally not recommended. Separate formulations of montelukast and levocetirizine are available for younger children, with specific dosing based on age and weight.
## Dose Adjustments
* **Renal Impairment:**

* **Neuropsychiatric effects (Montelukast):** Increased risk of behavioral and mood-related changes including aggression, depression, anxiety, hallucinations, insomnia, and suicidal thoughts. Patients and caregivers should be informed of these potential effects.
## Key Drug Interactions
* **CNS Depressants (e.g., sedatives, benzodiazepines, opioids):** Additive CNS depressant effects with levocetirizine.
* **Theophylline:** May decrease the clearance of theophylline, increasing theophylline serum concentrations. Monitor theophylline levels.
* **CYP1A2 Inhibitors (e.g., fluvoxamine, ciprofloxacin):** May increase montelukast levels.
## Monitoring
* Monitor for efficacy in symptom control for allergic rhinitis and urticaria.
* Assess for adverse effects, particularly neuropsychiatric changes with montelukast and somnolence with levocetirizine.
## Clinical Pearls
* This fixed-dose combination offers convenience for patients requiring both agents.
* Montelukast carries a boxed warning regarding neuropsychiatric events; patient and caregiver counseling is critical.
* Levocetirizine can cause somnolence; advise patients to use caution when driving or operating machinery.
* Administer montelukast in the evening, especially for patients with exercise-induced bronchoconstriction (though this combination is not indicated for asthma therapy).
