# Montelukast-Levocetirizine
## Overview
This combination provides a leukotriene receptor antagonist (montelukast) and a second-generation antihistamine (levocetirizine) for managing allergic respiratory conditions.
## Primary Indications
* Allergic rhinitis (seasonal and perennial)
* Asthma (add-on therapy)
* Chronic urticaria
## Adult Dosing
* **Montelukast:** 10 mg orally once daily in the evening.
* **Levocetirizine:** 5 mg orally once daily, may be taken in the evening.
* **Combination:** Typically available as a fixed-dose combination tablet containing montelukast 10 mg and levocetirizine 5 mg.
## Pediatric Dosing
Dosing is weight-based and age-dependent for montelukast and levocetirizine individually.
* **Montelukast:**
* 1 to 5 years: 4 mg orally once daily.
* 6 to 14 years: 5 mg orally once daily.
* 15 years and older: 10 mg orally once daily.
* **Levocetirizine:**
* 6 to 11 months: 1.25 mg orally once daily.
* 1 to 5 years: 1.25 mg orally twice daily.
* 6 to 11 years: 2.5 mg orally once daily.
* 12 years and older: 5 mg orally once daily.
* **Combination:** Fixed-dose combination products are generally not available for pediatric use below 12 years of age. Separate formulations should be used. Consult product-specific labeling for children younger than 6 months for levocetirizine.
## Dose Adjustments
* **Hepatic Impairment:** Use with caution. Mild to moderate hepatic impairment may require dose reduction of levocetirizine. Montelukast does not typically require dose adjustment in mild to moderate hepatic impairment, but caution is advised. Severe hepatic impairment is a contraindication for levocetirizine.
* **Renal Impairment:**
* **Levocetirizine:** Reduce dose in moderate to severe renal impairment. Creatinine clearance (CrCl) 31-50 mL/min: 2.5 mg once daily. CrCl < 30 mL/min: 2.5 mg every other day. CrCl < 10 mL/min: Use is not recommended.
* **Montelukast:** Not typically required.
## Contraindications
* Hypersensitivity to montelukast, levocetirizine, or any component of the formulation.
* Severe hepatic impairment (for levocetirizine).
## Adverse Effects
* **Montelukast:** Headache, abdominal pain, diarrhea, nausea, vomiting, rash. Increased risk of neuropsychiatric effects (e.g., mood changes, suicidal ideation, sleep disturbances).
* **Levocetirizine:** Somnolence, dry mouth, fatigue, pharyngitis, dizziness.
* **Combination:** A combination of the above. Somnolence is a significant concern with levocetirizine, especially at higher doses or in combination.
## Key Drug Interactions
* **Levocetirizine:** Increased CNS depressant effects with alcohol and other CNS depressants (e.g., benzodiazepines, opioids). Theophylline may reduce levocetirizine clearance.
* **Montelukast:** Grapefruit juice may increase montelukast levels, but clinical significance is uncertain. Phenobarbital and rifampin may decrease montelukast levels.
## Monitoring
* Monitor for therapeutic effectiveness.
* Monitor for adverse effects, particularly neuropsychiatric changes with montelukast and somnolence with levocetirizine.
* Assess renal function for levocetirizine dose adjustments.
* Assess hepatic function for levocetirizine dose adjustments.
## Clinical Pearls
* Montelukast should be taken in the evening, regardless of mealtime.
* Levocetirizine can be taken with or without food. Evening dosing for levocetirizine may help mitigate daytime somnolence.
* The FDA has issued a boxed warning regarding serious neuropsychiatric events associated with montelukast; patients and caregivers should be informed of these risks.
* While often used for allergic rhinitis, montelukast's role in asthma is as an add-on therapy, not a first-line treatment.
* Separate formulations should be used for pediatric patients, especially those under 12 years, as fixed-dose combination products may not be available or appropriate.
