### Primary Indications
* Allergic rhinitis (seasonal and perennial)
* Chronic idiopathic urticaria
### Adult Dosing

* **Chronic Idiopathic Urticaria:** One tablet (montelukast 10 mg / levocetirizine 5 mg) once daily.
### Pediatric Dosing
Dosing for pediatric patients is typically weight-based and formulation-dependent (e.g., chewable tablets, oral solution). Specific combination products may not be available for all pediatric age groups. Consult product-specific prescribing information.
* **Montelukast (as component):**
* 1-5 years: 4 mg orally once daily.
* 6-14 years: 5 mg orally once daily.
* $\geq$15 years: 10 mg orally once daily.

*Note: Fixed-dose combination product availability and dosing in pediatrics can vary. Check specific product labeling.*
### Dose Adjustments
* **Hepatic Impairment:** Montelukast dose may need to be reduced in severe hepatic impairment. Levocetirizine dose should be reduced in moderate to severe hepatic impairment. Consult specific product labeling and local guidelines for precise dose adjustments.
### Contraindications
* Hypersensitivity to montelukast, levocetirizine, or any component of the formulation.
* Severe hepatic impairment (for montelukast component).
### Adverse Effects
* **Common:** Dizziness, fatigue, somnolence, cough, nasopharyngitis, upper respiratory tract infection, headache, abdominal pain, diarrhea, rash.
* **Neuropsychiatric effects (Montelukast):** Behavioral and mood changes, including agitation, aggression, depression, dreams, insomnia, irritability, suicidal thoughts. **Black box warning exists for montelukast regarding neuropsychiatric events.**
* **Hypersensitivity reactions (Levocetirizine):** Angioedema, anaphylaxis, rash, urticaria.
### Key Drug Interactions
* **CYP2C8 inducers (e.g., rifampin, gemfibrozil):** May increase montelukast exposure; monitor for adverse effects.
* **CNS depressants (e.g., benzodiazepines, opioids, sedating antihistamines):** Additive sedative effects may occur with levocetirizine.
* **Theophylline:** Montelukast may increase theophylline serum concentrations; monitor theophylline levels.
### Monitoring
* Monitor for behavioral and mood changes, especially in patients with pre-existing psychiatric conditions. Report any concerning changes to the prescriber immediately.
* Monitor for signs of hypersensitivity reactions.
* Assess efficacy in controlling allergic rhinitis or urticaria symptoms.
### Clinical Pearls
* Montelukast/levocetirizine is generally taken once daily, in the evening or anytime, with or without food.
* Due to the potential for somnolence with levocetirizine, caution patients about activities requiring mental alertness.
* The neuropsychiatric effects associated with montelukast warrant careful patient and caregiver counseling and education regarding potential behavioral and mood changes.
* This combination is not typically a first-line therapy for acute asthma exacerbations or status asthmaticus.
