# Miconazole Vaginal Pessary
## Overview
Miconazole is an imidazole antifungal agent that inhibits the synthesis of ergosterol, a vital component of fungal cell membranes.
## Primary Indications
Treatment of vulvovaginal candidiasis (yeast infection).
## Adult Dosing
* **100 mg vaginal pessary:** Insert one pessary vaginally once daily at bedtime for 7 days.
* **200 mg vaginal pessary:** Insert one pessary vaginally once daily at bedtime for 3 days.
* **150 mg vaginal pessary (single dose):** Insert one pessary vaginally at bedtime.
## Pediatric Dosing
* **Under 12 years:** Safety and efficacy have not been established. Use in this population is generally not recommended.
* **12 years and older:** Dosing is the same as adult dosing.
## Dose Adjustments
No dose adjustments are necessary for renal or hepatic impairment.
## Contraindications
Hypersensitivity to miconazole or any component of the formulation.
## Adverse Effects
Common adverse effects include vaginal burning, itching, irritation, and abdominal cramps. Vulvar or vaginal redness, swelling, and rash may also occur.
## Key Drug Interactions
* **Warfarin:** Miconazole can inhibit the metabolism of warfarin, increasing the anticoagulant effect. Monitor INR closely if coadministered.
* **CYP3A4 substrates:** Miconazole is a moderate inhibitor of CYP3A4. Concomitant use with drugs metabolized by CYP3A4 may increase plasma concentrations of these drugs.
## Monitoring
* Clinical improvement of symptoms (e.g., reduced itching, burning, discharge).
* For patients on warfarin, regular INR monitoring is crucial.
## Clinical Pearls
* Use the provided applicator for insertion.
* Complete the full course of treatment, even if symptoms improve earlier, to ensure effective eradication of the fungus.
* Avoid intercourse during treatment as it could potentially be transmitted to a partner or reduce the efficacy of the medication.
* Alcohol-based lubricants may interact with latex condoms and diaphragms, potentially causing them to break down.
