# Miconazole Vaginal Pessary
## Overview
Miconazole is an imidazole antifungal agent that inhibits the synthesis of ergosterol, a vital component of fungal cell membranes, leading to increased membrane permeability and cell death.
## Primary Indications
Treatment of vulvovaginal candidiasis (yeast infections).
## Adult Dosing
Several formulations are available:
* **1-day treatment:** 1200 mg vaginal suppository inserted at bedtime.
* **3-day treatment:** 200 mg vaginal suppository inserted at bedtime for 3 consecutive nights.
* **7-day treatment:** 100 mg vaginal suppository inserted at bedtime for 7 consecutive nights.
The choice of regimen may depend on patient preference, symptom severity, and local protocols.
## Pediatric Dosing
Safety and efficacy in pediatric patients are not well-established. Use is generally not recommended in children younger than 12 years due to lack of data.
## Dose Adjustments
No dose adjustments are typically required for renal or hepatic impairment.
## Contraindications
Known hypersensitivity to miconazole or any component of the formulation.
## Adverse Effects
* **Common:** Vulvovaginal burning, itching, irritation, redness, and abdominal cramps.
* **Less Common:** Headache, rash.
## Key Drug Interactions
* **Warfarin:** Miconazole can inhibit the metabolism of warfarin, potentially increasing the risk of bleeding. Close monitoring of INR is recommended if co-administered.
* **CYP3A4 Substrates:** Miconazole may inhibit CYP3A4, potentially increasing plasma concentrations of other drugs metabolized by this enzyme.
## Monitoring
* Assess for symptom improvement.
* Monitor INR if co-administered with warfarin.
## Clinical Pearls
* Complete the full course of treatment, even if symptoms improve earlier.
* Avoid intercourse during treatment as the miconazole product may weaken latex condoms and diaphragms.
* If symptoms persist or worsen after completing treatment, consult a healthcare provider to rule out other causes or resistant organisms.
