# Miconazole Vaginal Pessary
## Overview
Miconazole is an imidazole antifungal agent. The vaginal pessary formulation is designed for localized treatment of vaginal yeast infections.
## Primary Indications
Treatment of vulvovaginal candidiasis (yeast infection).
## Adult Dosing
* **100 mg vaginal pessary:** Insert 1 pessary vaginally once daily at bedtime for 7 consecutive days.
* **200 mg vaginal pessary:** Insert 1 pessary vaginally once daily at bedtime for 3 consecutive days.
* **150 mg vaginal pessary:** Insert 1 pessary vaginally once daily at bedtime for a single dose.
## Pediatric Dosing
Dosing for pediatric patients is not well established and should be based on clinical judgment and consultation with pediatric specialists. Local protocols may provide guidance.
## Dose Adjustments
No specific dose adjustments are necessary for renal or hepatic impairment due to minimal systemic absorption.
## Contraindications
* Hypersensitivity to miconazole or any component of the pessary formulation.
## Adverse Effects
Common adverse effects include vaginal irritation, burning, itching, and pelvic cramps. Less common: rash, hives.
## Key Drug Interactions
Minimal systemic absorption limits significant drug interactions. However, caution is advised if used concurrently with warfarin, as miconazole can inhibit CYP2C9 and increase warfarin levels; monitor INR closely.
## Monitoring
* Symptomatic improvement (e.g., resolution of itching, burning, discharge).
* Adverse effect monitoring.
## Clinical Pearls
* Ensure the patient understands proper insertion technique for optimal efficacy.
* Complete the full course of treatment even if symptoms improve earlier.
* Avoid concurrent use of spermicides due to potential inactivation.
