# Miconazole Vaginal Pessary
## Overview
Miconazole is an imidazole antifungal agent that inhibits the synthesis of ergosterol, a key component of fungal cell membranes.
## Primary Indications
Treatment of vulvovaginal candidiasis (yeast infection).
## Adult Dosing
* **100 mg vaginal pessary:** Insert one pessary vaginally once daily at bedtime for 7 days.
* **200 mg vaginal pessary:** Insert one pessary vaginally once daily at bedtime for 3 days.
* **Higher strength formulations (e.g., 1200 mg single dose) may also be available, follow specific product labeling.**
## Pediatric Dosing
Data for pediatric use of miconazole vaginal pessaries is limited. Use should be based on clinical judgment and may require alternative formulations or specific physician guidance.
## Dose Adjustments
No dose adjustments are typically required for renal or hepatic impairment.
## Contraindications
* Hypersensitivity to miconazole or any component of the formulation.
* Concomitant use with CYP3A4 inhibitors (see Key Drug Interactions).
## Adverse Effects
* **Common:** Vaginal burning, itching, irritation, pelvic cramps, headache.
* **Less Common:** Allergic reactions, rash.
## Key Drug Interactions
* **CYP3A4 Inhibitors:** Miconazole is a potent inhibitor of CYP3A4. Concomitant use with CYP3A4 substrates (e.g., certain statins, benzodiazepines, anticoagulants like warfarin) may lead to increased plasma concentrations of these drugs, potentially causing serious adverse events. Avoid concomitant use or monitor closely.
## Monitoring
* Monitor for symptomatic improvement.
* Discontinue use if significant irritation or allergic reaction occurs.
## Clinical Pearls
* Advise patients to complete the full course of treatment, even if symptoms improve earlier.
* Avoid intercourse during treatment as the pessary may damage latex condoms and diaphragms, potentially leading to unintended pregnancy or STI transmission.
* Some formulations may be oily and could stain underwear; patient counseling on appropriate protection is advised.
* If symptoms do not improve after completion of therapy, or if they recur, re-evaluate the diagnosis and consider alternative treatment.
