# Miconazole Vaginal Pessary
## Overview
Miconazole is an imidazole antifungal that inhibits the synthesis of ergosterol, a vital component of fungal cell membranes. It is primarily used for the treatment of vulvovaginal candidiasis.
## Primary Indications
* Vulvovaginal candidiasis (yeast infection).
## Adult Dosing
* **100 mg pessary:** 1 pessary inserted intravaginally once daily at bedtime for 7 consecutive days.
* **200 mg pessary:** 1 pessary inserted intravaginally once daily at bedtime for 3 consecutive days.
* **1200 mg pessary:** 1 pessary inserted intravaginally as a single dose at bedtime.
## Pediatric Dosing
* **Children < 12 years:** Use is not recommended without clinical evaluation by a physician to rule out other causes of vulvovaginitis.
* **Adolescents (≥ 12 years):** Same as adult dosing.
## Dose Adjustments
* **Renal/Hepatic Impairment:** No systemic dose adjustment required due to minimal systemic absorption.
## Contraindications
* Hypersensitivity to miconazole or any component of the formulation.
* History of recurrent yeast infections (greater than 3 episodes in 12 months) requires formal diagnosis.
## Adverse Effects
* **Local/Genital:** Vulvovaginal burning, itching, irritation, or pelvic cramping.
* **Dermatologic:** Skin rash or urticaria (rare).
* **Systemic:** Extremely rare due to negligible absorption, but watch for signs of hypersensitivity.
## Key Drug Interactions
* **Warfarin:** Although systemic absorption is low, systemic azole antifungals can inhibit CYP2C9 and CYP3A4, potentially increasing the International Normalized Ratio (INR). Monitor closely if the patient is on warfarin therapy.
## Monitoring
* **Efficacy:** Resolution of symptoms (itching, discharge, burning).
* **Safety:** Monitor for local irritation or allergic contact dermatitis. If symptoms persist after the recommended course, discontinue use and evaluate for alternative diagnoses (e.g., bacterial vaginosis, trichomoniasis).
## Clinical Pearls
* **Compliance:** Complete the full course of therapy even if symptoms resolve earlier to ensure mycological cure.
* **Barrier Contraception:** Miconazole can damage latex condoms and diaphragms; advise patients to use alternative contraception or abstain from intercourse during treatment.
* **Administration:** Insert as high into the vagina as possible using the provided applicator or fingers.
* **Menstruation:** Therapy should not be interrupted if menstruation begins; use of sanitary pads is recommended (avoid tampons).
