# Miconazole vaginal pessary
## Overview
Miconazole nitrate is an imidazole antifungal agent that inhibits the synthesis of ergosterol, a vital component of fungal cell membranes. The vaginal pessary (suppository) form is used for local treatment of vulvovaginal candidiasis.
## Primary Indications
* Vulvovaginal candidiasis (yeast infection).
## Adult Dosing
* **100 mg pessary:** Insert 1 pessary into the vagina once daily at bedtime for 7 consecutive days.
* **200 mg pessary:** Insert 1 pessary into the vagina once daily at bedtime for 3 consecutive days.
* **1200 mg pessary:** Insert 1 pessary into the vagina as a single dose at bedtime.
## Pediatric Dosing
* **Adolescents (≥12 years):** Same as adult dosing.
* **Children (<12 years):** Not recommended without formal clinical evaluation to rule out other causes of vaginitis. Safety and efficacy not established.
## Dose Adjustments
* **Renal/Hepatic Impairment:** No specific adjustments required due to minimal systemic absorption.
## Contraindications
* Hypersensitivity to miconazole or any component of the formulation.
* Recurrent symptoms (e.g., >3 infections per year) require clinical evaluation to rule out underlying conditions (e.g., diabetes, immune suppression).
## Adverse Effects
* **Local:** Vaginal burning, itching, irritation, or pelvic cramping.
* **Systemic:** Very rare; contact hypersensitivity or headache.
## Key Drug Interactions
* **Warfarin:** Although systemic absorption is minimal, monitoring of INR is advised if significant mucosal inflammation increases systemic uptake, as miconazole can inhibit CYP2C9 and increase warfarin levels.
* **Latex/Diaphragms:** Miconazole can damage latex condoms and diaphragms; avoid use of these products during treatment and for 3 days following the final dose.
## Monitoring
* Monitor for worsening local irritation.
* If symptoms persist after completion of the course, discontinue and reassess for resistant strains or alternative diagnoses (e.g., bacterial vaginosis, trichomoniasis).
## Clinical Pearls
* **Adherence:** Complete the full course of treatment even if symptoms resolve earlier to prevent recurrence.
* **Administration:** Pessaries are designed for intravaginal use only. Advise patients to insert the dose high into the vagina using the provided applicator or as instructed.
* **Hygiene:** Advise patients to avoid wearing tampons, douching, or using vaginal sprays during treatment.
* **Refractory Cases:** Recurrent or persistent symptoms require a physical examination and laboratory confirmation (e.g., wet mount, fungal culture) to exclude non-albicans *Candida* species which may not respond to miconazole.
