# Miconazole Vaginal Pessary
## Overview
Miconazole is an azole antifungal that inhibits the synthesis of ergosterol, a vital component of the fungal cell membrane, leading to cell death. Vaginal pessaries (suppositories) are commonly used for the treatment of vulvovaginal candidiasis.
## Primary Indications
Treatment of vulvovaginal candidiasis (yeast infections).
## Adult Dosing
* **100 mg pessary:** Insert 1 pessary intravaginally daily at bedtime for 7 consecutive days.
* **200 mg pessary:** Insert 1 pessary intravaginally daily at bedtime for 3 consecutive days.
* **1200 mg pessary:** Insert 1 pessary intravaginally as a single one-time dose at bedtime.
## Pediatric Dosing
Safety and efficacy in pediatric patients (<12 years) have not been established. Use in adolescents should be supervised by a healthcare provider to confirm diagnosis.
## Dose Adjustments
No systemic dose adjustments are required due to low systemic absorption. If symptoms persist or recur within 2 months, reconsider the diagnosis.
## Contraindications
* Hypersensitivity to miconazole or any component of the formulation.
* Known sensitivity to other imidazole antifungals.
## Adverse Effects
* **Common:** Local burning, itching, irritation, or pelvic cramping.
* **Rare:** Contact dermatitis, headache, or allergic reaction (e.g., rash, swelling).
## Key Drug Interactions
