# Mega-CV Duo (Amoxicillin/Clavulanate Potassium)
## Overview
Mega-CV Duo is a fixed-dose combination antibiotic consisting of amoxicillin (a beta-lactam) and clavulanate potassium (a beta-lactamase inhibitor). The clavulanate component expands the spectrum of activity to include beta-lactamase-producing bacteria that are otherwise resistant to amoxicillin alone.
## Primary Indications
* Community-acquired pneumonia
* Acute bacterial sinusitis
* Acute otitis media
* Skin and skin structure infections
* Urinary tract infections
* Lower respiratory tract infections
## Adult Dosing
Standard dosing is based on the amoxicillin component.
* **Mild to Moderate Infections:** 500 mg amoxicillin/125 mg clavulanate every 8 hours or 875 mg/125 mg every 12 hours.
* **Severe Infections:** 875 mg/125 mg every 8 to 12 hours.
* **Maximum Dose:** The clavulanate component limits the maximum dose to avoid hepatotoxicity and GI distress. Do not exceed 875 mg amoxicillin/125 mg clavulanate per dose.
## Pediatric Dosing
Dosing is strictly weight-based (calculated as mg of amoxicillin component).
* **General Recommendation:** 20 to 45 mg/kg/day in divided doses every 8–12 hours, depending on the severity of the infection.
* **Recurrent/Resistant Otitis Media:** 80–90 mg/kg/day divided every 12 hours.
* *Note:* Always verify the ratio of the formulation provided (e.g., 200/28, 400/57, or 600/42.9 mg/5 mL) to ensure the correct clavulanate dose is administered, as some concentrations are not interchangeable.
## Dose Adjustments
* **Renal Impairment:** Reduce frequency for CrCl < 30 mL/min. Avoid the 875 mg strength in patients with severe renal impairment (CrCl < 30 mL/min) because the clavulanate load cannot be adjusted independently.
* **Hepatic Impairment:** Use with caution; monitor hepatic function regularly.
## Contraindications
* Known hypersensitivity to amoxicillin, clavulanate, or other penicillins/beta-lactams.
* History of cholestatic jaundice or hepatic dysfunction associated with previous amoxicillin/clavulanate therapy.
## Adverse Effects
* **Common:** Diarrhea (often dose-related due to clavulanate), nausea, vomiting, abdominal pain, skin rash/urticaria.
* **Serious:** Clostridioides difficile-associated diarrhea (CDAD), hepatotoxicity (cholestatic jaundice), anaphylaxis, Stevens-Johnson syndrome.
## Key Drug Interactions
* **Probenecid:** Decreases renal tubular secretion of amoxicillin, increasing serum levels.
* **Oral Contraceptives:** Potential for decreased efficacy (evidence base is debated, but barrier protection is often advised).
* **Warfarin:** May increase INR and risk of bleeding; monitor PT/INR closely when initiating or stopping therapy.
* **Methotrexate:** May decrease renal clearance of methotrexate, increasing risk of toxicity.
## Monitoring
* Monitor for signs of anaphylaxis during the first dose.
* Evaluate LFTs if therapy duration exceeds 14 days.
* Monitor for signs of CDAD (persistent watery or bloody diarrhea).
* Monitor renal function in patients with pre-existing impairment.
## Clinical Pearls
* **GI Tolerance:** Administer at the start of a meal to minimize GI upset and improve absorption.
* **Diarrhea Management:** If diarrhea is dose-limiting, consider switching products or assessing if the clavulanate ratio is inappropriate for the patient's age.
* **Formulation Caution:** Not all strengths are bioequivalent. Always calculate based on the specific mg of amoxicillin provided in the local formulary to prevent toxicity or under-dosing.
