# Mega CV
## Overview
Mega CV is a brand-name formulation typically containing a combination of **Amoxicillin and Clavulanate Potassium**. It is a penicillin-class antibiotic combined with a beta-lactamase inhibitor to broaden the spectrum of activity against beta-lactamase-producing bacteria.
## Primary Indications
Treatment of infections caused by susceptible organisms, including:
* Acute bacterial sinusitis
* Community-acquired pneumonia
* Skin and skin structure infections
* Urinary tract infections
* Otitis media (pediatric)
## Adult Dosing
* **Standard dosing (based on amoxicillin component):** 500 mg every 8 or 12 hours, or 875 mg every 12 hours.
* **Extended-release tablets:** 2000 mg every 12 hours (must swallow whole; high-dose amoxicillin component).
* **Maximum:** Dependent on the specific formulation; generally limited by the clavulanate component to avoid gastrointestinal toxicity.
## Pediatric Dosing
* **Standard dose:** 20–45 mg/kg/day (based on amoxicillin component) divided every 8–12 hours.
* **Severe infection/Otitis media:** Up to 90 mg/kg/day (amoxicillin component) divided every 12 hours.
* **Guidance Note:** Always verify the specific concentration of the liquid suspension (e.g., 200 mg/5 mL vs 600 mg/5 mL) to prevent dosing errors.
## Dose Adjustments
* **Renal Impairment:** Dosage must be adjusted based on creatinine clearance (CrCl).
* **CrCl <30 mL/min:** Extended-release formulations are contraindicated.
* **CrCl 10–30 mL/min:** Use 500 mg every 12 hours. Avoid 875 mg tablets.
* **CrCl <10 mL/min:** 500 mg every 24 hours.
## Contraindications
* History of serious allergic reaction (anaphylaxis) to any penicillin.
* History of cholestatic jaundice or hepatic dysfunction associated with previous use of amoxicillin/clavulanate.
## Adverse Effects
* **Common:** Diarrhea (frequently associated with the clavulanate component), nausea, vomiting, skin rash.
* **Serious:** *Clostridioides difficile*-associated diarrhea, hepatotoxicity (cholestatic jaundice), severe hypersensitivity reactions (Stevens-Johnson syndrome).
## Key Drug Interactions
* **Warfarin:** May increase INR/risk of bleeding; monitor PT/INR.
* **Methotrexate:** May decrease renal clearance, increasing systemic toxicity risk.
* **Oral Contraceptives:** May reduce efficacy; backup contraception is recommended during therapy.
* **Probenecid:** Increases and prolongs amoxicillin plasma levels.
## Monitoring
* Monitor for signs of hypersensitivity (rash, urticaria, wheezing).
* Assess for persistent diarrhea (suspect *C. difficile*).
* Monitor liver function tests (LFTs) periodically in patients on long-term therapy.
* Monitor renal function in patients with baseline impairment.
## Clinical Pearls
* **Dosing Ratio:** Always confirm the amoxicillin-to-clavulanate ratio. Formulations with 875 mg of amoxicillin often contain 125 mg of clavulanate.
* **Administration:** Take at the start of a meal to minimize gastrointestinal upset and maximize absorption.
* **Suspension:** Must be stored in the refrigerator; discard after 10 days.
