# Mega CV (Amoxicillin/Clavulanate)
## Overview
Mega CV is a combination antibiotic consisting of amoxicillin (a beta-lactam penicillin) and potassium clavulanate (a beta-lactamase inhibitor). It broadens the spectrum of amoxicillin to cover beta-lactamase-producing bacteria (e.g., *Staphylococcus aureus*, *H. influenzae*, *M. catarrhalis*).
## Primary Indications
* Acute bacterial rhinosinusitis.
* Community-acquired pneumonia.
* Acute otitis media.
* Skin and skin structure infections.
* Urinary tract infections.
* Human or animal bite prophylaxis/treatment.
## Adult Dosing
* **Standard dosing:** 500 mg/125 mg orally every 8 hours or 875 mg/125 mg orally every 12 hours.
* **Severe infections/Respiratory tract infections:** 875 mg/125 mg orally every 12 hours or 2000 mg (extended-release) every 12 hours.
* **Max dose:** Do not exceed 2000 mg of amoxicillin component per dose to limit clavulanate-associated gastrointestinal side effects.
## Pediatric Dosing
* **Dosing is weight-based:** Typically 20 mg/kg/day to 45 mg/kg/day (amoxicillin component) divided every 8 to 12 hours depending on the severity and type of infection.
* **Recurrent/Resistant Otitis Media:** Often dosed at 90 mg/kg/day (amoxicillin component) divided every 12 hours using the 4:1 or 7:1 ratio formulations.
* **Important:** Dose must be based on the specific concentration ratio of the product to avoid excessive clavulanate intake.
## Dose Adjustments
* **Renal Impairment:** Adjustments required for CrCl < 30 mL/min. The 875 mg tablet should not be used in patients with severe renal impairment (CrCl < 30 mL/min).
* **Hepatic Impairment:** Use with caution; monitor hepatic function periodically.
## Contraindications
* History of serious hypersensitivity (e.g., anaphylaxis, Stevens-Johnson syndrome) to amoxicillin, other penicillins, or cephalosporins.
* History of cholestatic jaundice or hepatic dysfunction associated with amoxicillin/clavulanate therapy.
## Adverse Effects
* **Common:** Diarrhea, nausea, vomiting, abdominal pain (often due to the clavulanate component).
* **Serious:** C. difficile-associated diarrhea (CDAD), hepatotoxicity (elevated LFTs, cholestatic jaundice), severe skin reactions, and hypersensitivity (anaphylaxis).
## Key Drug Interactions
* **Warfarin:** May increase INR and bleeding risk due to alteration of gut flora and potential Vitamin K synthesis interference.
* **Methotrexate:** Penicillins may decrease renal clearance of methotrexate, increasing toxicity risk.
* **Oral Contraceptives:** May reduce efficacy (though clinical evidence is debated, back-up contraception is often recommended).
* **Probenecid:** Decreases renal excretion of amoxicillin, increasing serum levels.
## Monitoring
* **Renal/Hepatic:** Periodic LFTs and renal function tests during prolonged therapy.
* **GI:** Monitor for signs of *C. difficile* infection (worsening diarrhea, abdominal cramping, fever).
* **Allergy:** Assess for signs of rash, angioedema, or respiratory distress.
## Clinical Pearls
* **Ratio Matters:** The ratio of amoxicillin to clavulanate varies by dosage form and strength. Always verify the ratio (e.g., 4:1 vs 7:1) to ensure the correct pediatric dose is administered.
* **Administration:** Take with food or a meal to minimize potential gastrointestinal discomfort.
* **Clavulanate Toxicity:** Gastrointestinal side effects are directly related to the amount of clavulanate; if diarrhea is severe, look for formulations with a lower proportion of clavulanic acid.
