# Mega CV
## Overview
Mega CV is a fixed-dose combination product containing **Amoxicillin** (a penicillin-class antibiotic) and **Clavulanate Potassium** (a beta-lactamase inhibitor). It acts by expanding the antibacterial spectrum of amoxicillin to cover beta-lactamase-producing organisms.
## Primary Indications
* Acute bacterial rhinosinusitis.
* Community-acquired pneumonia.
* Acute otitis media.
* Skin and skin structure infections.
* Urinary tract infections.
## Adult Dosing
Standard dosing is based on the amoxicillin component:
* **Mild to Moderate Infections:** 500 mg amoxicillin/125 mg clavulanate every 8 hours OR 875 mg amoxicillin/125 mg clavulanate every 12 hours.
* **Severe Infections/Respiratory Tract Infections:** 875 mg amoxicillin/125 mg clavulanate every 12 hours.
* **Extended Release (ER):** 2000 mg/125 mg every 12 hours (only for specific indications like CAP or sinusitis).
## Pediatric Dosing
**Note:** Dosing is weight-based. Ensure the correct formulation is selected to avoid excessive clavulanate intake.
* **General infection:** 20 mg/kg/day to 45 mg/kg/day (based on amoxicillin component) divided every 12 hours.
* **Severe infection/Otitis Media:** 80 mg/kg/day to 90 mg/kg/day divided every 12 hours using the 600 mg/5 mL suspension (to maintain appropriate ratios).
* **Maximum:** Follow specific weight-based pediatric guidelines; do not exceed adult maximums.
## Dose Adjustments
* **Renal Impairment:** Reduce frequency if CrCl <30 mL/min. Do not use the 875 mg tablet in patients with severe renal impairment (CrCl <30 mL/min).
* **Hepatic Impairment:** Use with caution; monitor hepatic function.
## Contraindications
* History of serious hypersensitivity (e.g., anaphylaxis) to penicillins or any component of the formulation.
* History of cholestatic jaundice or hepatic dysfunction associated with previous use of amoxicillin/clavulanate.
## Adverse Effects
* **Common:** Diarrhea (frequent due to clavulanate), nausea, vomiting, abdominal pain, skin rash.
* **Serious:** *Clostridioides difficile*-associated diarrhea, severe hypersensitivity reactions (anaphylaxis, SJS/TEN), and idiosyncratic hepatotoxicity (cholestatic jaundice).
## Key Drug Interactions
* **Methotrexate:** Amoxicillin may decrease the renal clearance of methotrexate, increasing toxicity.
* **Warfarin:** May increase the anticoagulant effect (monitor INR).
* **Oral Contraceptives:** Potential for reduced efficacy (though evidence is limited, backup contraception is often recommended).
* **Probenecid:** Increases and prolongs amoxicillin serum levels.
## Monitoring
* Monitor for signs of hypersensitivity.
* Assess renal and hepatic function during prolonged therapy.
* Observe for signs of secondary superinfection (e.g., *C. difficile*).
## Clinical Pearls
* **Administration:** Take with food or a meal to minimize gastrointestinal upset.
* **Formulation Warning:** The 250 mg and 500 mg tablets contain the same amount of clavulanate (125 mg). Do not substitute two 250 mg tablets for one 500 mg tablet to avoid excessive clavulanate exposure.
* **Dosing Ratio:** Always confirm the amoxicillin:clavulanate ratio (e.g., 4:1 vs 7:1) when calculating pediatric doses to prevent toxicity.
* **Uncertainty:** Dosing may vary based on local antibiograms; always consult institutional guidelines for empiric therapy.
