## Overview
Meftal-Spas is a fixed-dose combination medication containing **Mefenamic Acid** (250 mg), an NSAID, and **Dicyclomine Hydrochloride** (10 mg), an antispasmodic/anticholinergic agent. It is designed to provide dual relief from smooth muscle spasms and prostaglandin-mediated pain.
## Primary Indications
* Management of spasmodic dysmenorrhea (primary).
* Functional bowel/GI disorders characterized by hypermotility and cramping.
* Musculoskeletal pain associated with smooth muscle spasm.
## Adult Dosing
* **Standard Dose:** 1 tablet (250 mg Mefenamic Acid / 10 mg Dicyclomine) taken 3 to 4 times daily.
* **Maximum Dose:** Do not exceed 4 tablets in 24 hours.
* **Duration:** Use for the shortest duration necessary; generally not recommended for more than 7 days.
## Pediatric Dosing
* **Children < 12 years:** Generally not recommended due to lack of established safety and efficacy data for this specific fixed-dose combination.
* **Adolescents (12+ years):** Same as adult dosing, though weight-based adjustment is preferred if possible. Use with caution.
## Dose Adjustments

* **Anticoagulants (e.g., Warfarin):** Increases risk of bleeding.
* **Corticosteroids/SSRIs:** Increases risk of GI ulceration.
* **Antihypertensives/Diuretics:** NSAIDs may reduce the natriuretic effect and decrease antihypertensive efficacy.
* **Other Anticholinergics:** Synergistic effects (increased risk of confusion and urinary retention).
* **Lithium/Methotrexate:** NSAIDs may increase plasma concentrations and toxicity.
## Monitoring

* Monitor for signs of GI bleeding (melena, hematemesis).

* Take with food or milk to minimize gastric irritation from the Mefenamic Acid.
* Patients should be cautioned against operating heavy machinery until the effect of the Dicyclomine (drowsiness/blurred vision) is known.
* Dicyclomine can decrease sweating; advise patients to avoid overheating or heavy exercise in hot environments.
