## Overview
Meftal-Spas is a fixed-dose combination medication containing **Mefenamic Acid** (an NSAID; 250 mg) and **Dicyclomine Hydrochloride** (an antispasmodic; 10 mg).
## Primary Indications
* Symptomatic relief of spasmodic dysmenorrhea (primary).
* Abdominal cramps and pain associated with gastrointestinal hypermotility.
* Musculoskeletal pain and mild-to-moderate inflammatory conditions.
## Adult Dosing
* **Standard Dose:** 1 tablet (250 mg Mefenamic Acid / 10 mg Dicyclomine) taken orally 3 to 4 times daily.
* **Maximum Dose:** Do not exceed 4 tablets in 24 hours. Limit therapy duration to the shortest period necessary (usually < 7 days).
## Pediatric Dosing
* **Safety Warning:** Generally not recommended for children under 12 years of age due to lack of established safety and efficacy data for this specific combination. Use in adolescents should be supervised by a clinician.
## Dose Adjustments
* **Renal Impairment:** Avoid in severe renal impairment. Use with extreme caution in mild-to-moderate impairment; monitor creatinine clearance and fluid balance.

* **Lithium/Methotrexate:** NSAIDs increase plasma concentrations/toxicity risk.
* **Antipsychotics/Tricyclic Antidepressants:** May potentiate anticholinergic effects of Dicyclomine.
* **Antihypertensives (ACE inhibitors/Diuretics):** NSAIDs may reduce therapeutic efficacy and increase risk of acute kidney injury.
## Monitoring
* Monitor for signs of GI bleeding (occult blood, anemia).
* Monitor renal function (BUN/SCr) in long-term or high-risk patients.
* Monitor blood pressure during treatment.
* Assess for anticholinergic toxicity (especially urinary retention in elderly patients).
## Clinical Pearls
* **Administration:** Take with food or milk to minimize gastric irritation.
* **Alcohol:** Avoid alcohol, as it may enhance sedation from Dicyclomine and gastric irritation from Mefenamic Acid.
