# Meftal-Spas
## Overview
Meftal-Spas is a fixed-dose combination medication containing **Mefenamic Acid** (an NSAID, 250 mg) and **Dicyclomine Hydrochloride** (an antispasmodic/anticholinergic, 10 mg). It is primarily used for the symptomatic relief of painful smooth muscle spasms and menstrual pain.
## Primary Indications
* Primary dysmenorrhea (painful menstruation).
* Abdominal cramps/spasms associated with Gastrointestinal (GI) disorders.
* Biliary/renal colic spasms (adjunctive).
## Adult Dosing
* **Standard dose:** 1 tablet (Mefenamic Acid 250 mg/Dicyclomine 10 mg) taken orally 3 to 4 times daily as needed for pain.
* **Maximum dose:** Do not exceed 4 tablets in 24 hours. Limit duration of treatment to 3–5 days unless directed by a physician.
## Pediatric Dosing
* **Safety/Efficacy:** Generally **not recommended** for children under 12 years of age.
* **Adolescents (12+ years):** Same as adult dosing, though weight-based considerations apply. Consult local pediatric guidelines or a specialist before initiation.
## Dose Adjustments
* **Renal Impairment:** Contraindicated in patients with moderate to severe renal impairment (CrCl < 30 mL/min).
* **Hepatic Impairment:** Use with extreme caution; avoid in severe hepatic insufficiency.
* **Elderly:** Increased risk of anticholinergic side effects (confusion, urinary retention). Start at the lowest effective dose.
## Contraindications
* Hypersensitivity to mefenamic acid, dicyclomine, or aspirin/NSAIDs.

* **Anticholinergic-related:** Dry mouth, blurred vision, tachycardia, urinary retention, constipation, dizziness, and sedation.
## Key Drug Interactions
* **Other NSAIDs/Corticosteroids:** Increased risk of GI ulceration and bleeding.
* **Anticoagulants (e.g., Warfarin):** Enhanced anticoagulant effect; monitor INR.
* **Antihypertensives (ACE inhibitors/Diuretics):** Decreased efficacy and increased risk of acute renal failure.
* **CNS Depressants:** Dicyclomine may potentiate the effects of alcohol or other sedative-hypnotics.
* **Digoxin:** Dicyclomine may increase oral bioavailability of digoxin.
## Monitoring
* Monitor blood pressure and renal function (BUN/Creatinine) during prolonged use.
* Assess for occult GI bleeding (stools, anemia symptoms).
* Monitor for signs of anticholinergic toxicity (urinary retention, severe confusion, tachycardia).
## Clinical Pearls
* **Take with food:** Always administer with or after food to mitigate gastric irritation from the mefenamic acid component.
* **Avoid in Elderly:** Due to the anticholinergic burden of dicyclomine, it is often listed on the Beers Criteria as potentially inappropriate for older adults.
* **Hydration:** Ensure adequate fluid intake to support renal perfusion.
* **Caution:** Advise patients to avoid operating heavy machinery if they experience dizziness or blurred vision from the dicyclomine component.
