# Meftal-Spas
## Overview
Meftal-Spas is a combination medication containing **Mefenamic Acid** (an NSAID, 250 mg) and **Dicyclomine Hydrochloride** (an antispasmodic/anticholinergic, 10 mg). It is used to provide dual relief from spasmodic pain and inflammation.
## Primary Indications
* Management of primary dysmenorrhea (menstrual cramps).
* Symptomatic relief of intestinal colic, biliary colic, and renal colic.
* Spasmodic pain associated with gastrointestinal or genitourinary smooth muscle hypermotility.
## Adult Dosing
* **Standard Dose:** 1 tablet (250 mg Mefenamic Acid / 10 mg Dicyclomine) taken orally 3 to 4 times daily.
* **Maximum Dose:** Do not exceed 4 tablets in 24 hours. Limit total duration of use to less than 7 days to minimize NSAID-related risks.
## Pediatric Dosing
* **Not recommended for children under 14 years of age.**
* Safety and efficacy have not been established in the pediatric population for this specific combination.
## Dose Adjustments
* **Renal Impairment:** Avoid use in patients with moderate to severe renal impairment (CrCl < 60 mL/min).
* **Hepatic Impairment:** Avoid use in patients with severe hepatic impairment.
* **Geriatric Patients:** Use with extreme caution. Older adults are at higher risk for NSAID-related GI bleeds and anticholinergic side effects (e.g., urinary retention, confusion).
## Contraindications
* Hypersensitivity to mefenamic acid, dicyclomine, or aspirin/NSAIDs.
* History of GI bleeding or perforation related to NSAID therapy.
* Active peptic ulcer disease or inflammatory bowel disease (ulcerative colitis/Crohn’s).
* Obstructive uropathy or gastrointestinal obstruction (due to dicyclomine).
* Severe heart failure.
* Perioperative use in the setting of coronary artery bypass graft (CABG) surgery.
* Glaucoma or myasthenia gravis.
## Adverse Effects
* **NSAID-related:** Epigastric pain, nausea, peptic ulceration, GI bleeding, increased blood pressure, and potential cardiovascular thrombotic events.
* **Dicyclomine-related:** Dry mouth, blurred vision, tachycardia, urinary retention, constipation, and dizziness.
* **Neurological:** Drowsiness and confusion (particularly in the elderly).
## Key Drug Interactions
* **Anticoagulants:** (e.g., Warfarin) Increased risk of bleeding.
* **Antihypertensives:** (e.g., ACE inhibitors, diuretics) Reduced efficacy and risk of acute renal failure.
* **Lithium/Methotrexate:** Potential for increased serum concentrations and toxicity.
* **Other Anticholinergics:** Additive effects increasing the risk of urinary retention and cognitive impairment.
## Monitoring
* Monitor renal function (creatinine) and blood pressure periodically.
* Observe for signs of GI bleeding (occult blood in stool, melena).
* Monitor for symptoms of anticholinergic toxicity (extreme dry mouth, urinary hesitancy, altered mental status).
## Clinical Pearls
* **Administration:** Take with food or milk to minimize gastric irritation.
* **Limit Duration:** Due to the Mefenamic Acid component, long-term use should be avoided due to the increased risk of gastrointestinal, renal, and cardiovascular adverse effects.
* **Avoid Alcohol:** Alcohol can potentiate the sedative effects of dicyclomine and increase the risk of gastric irritation.
* **Caution:** Advise patients to avoid operating heavy machinery if drowsiness occurs.
